CELEM GŁÓWNYM PROJEKTU jest wzrost świadomości wśród 2.350 mieszkańców m. Radom i gm. Jedlnia-Letnisko i Gózd (w tym 1.175 M i 1.175 K) związanych z zagrożeniami jakie niesie cukrzyca i objęcie ich konsultacjami lekarskimi w kierunku wykrywania cukrzycy do 30.08.2019r. Planowane zadania są zgodne z założeniami Regionalnego Programu Zdrowotnego Samorządu Województwa Mazowieckiego (RPZ) i obejmują: akcję informacyjną (moduł I), konsultacje lekarskie i działania edukacyjne (moduł II) i szkolenia personelu medycznego (moduł III). GŁÓWNI UCZESTNICY PROJEKTU objęci wsparciem bezpośrednim to osoby pow. 60 r.ż. mieszkające w Radomiu i gm. Jedlnia-Letnisko i Gózd, dotychczas niediagnozowane w kierunku cukrzycy, które podpiszą druk świadomej zgody na udział w projekcie. Ponadto działaniami zostaną objęci również lekarze/pielęgniarki, personel placówek prowadzących działalność leczniczą. ZAKŁADANY WSKAŹNIK REZULTATU: liczba wspartych w programie miejsc świadczenia usług zdrowotnych, istniejących po zakończeniu projektu. ZAKŁADANE WSKAŹNIKI PRODUKTU: liczba osób zagrożonych ubóstwem lub wykluczeniem społecznym objętych usługami zdrowotnymi w programie, liczba personelu medycznego uczestniczącego w szkoleniach oraz liczba wykładów dla pacjentów dotyczące w szczególności diety i aktywności fizycznej. Pozostałe wskaźniki określone w RPZ będą wykazane i raportowane zgodnie z §30 pkt 3 wzoru umowy ryczałtowej dla przedmiotowego konkursu. Wnioskodawca we wskaźniku rezultatu nr 2 oraz wskaźnikach produktu nr 3, 4, 5, 6, 7, i 8 wskazał wartości docelowe na poziomie "0" - ich oszacowanie jest możliwe wyłącznie na etapie realizacji proj. i będzie raportowane zgodnie z zapisami umowy o dofinasowanie - informacje zostaną przekazane w terminie określonym w §30 pkt 3 wzoru umowy ryczałtowej. The main objective of the project is to increase awareness among the 2,350 inhabitants of Radom and Gm. Jedlnia-Letnisko and Gózd (including 1.175 M and 1.175 K) associated with the risks of diabetes and their medical consultation on diabetes detection by 30.08.2019. The planned tasks are consistent with the assumptions of the Regional Health Programme of the Mazovian Voivodship (RPZ) and include: information campaign (Module I), medical consultations and educational activities (module II) and training of medical staff (module III). The main beneficiaries of the project are persons covered by direct support. 60 years old, living in Radom and the building of Jedlnia-Letnisko and Gozd, not yet diagnosed in the direction of diabetes, which will sign the printing of informed consent to participate in the project. In addition, the activities will also include doctors/nurses, staff of medical establishments. EXPECTED RESULT INDICATOR: number of post-project health services supported by the programme. EXPECTED OUTPUT INDICATORS: the number of people at risk of poverty or social exclusion covered by health services in the programme, the number of medical staff participating in the training and the number of lectures for patients on diet and physical activity in particular. Other indicators specified in RPZ will be shown and reported in accordance with §30 point 3 of the model flat-rate agreement for the contest. The applicant in the result index No 2 and the Product Indicators No 3, 4, 5, 6, 7, and 8 indicated the target values at the level “0” – their estimation is possible only at the stage of the project implementation and will be reported in accordance with the provisions of the financing agreement – the information will be provided within the time limit specified in §30 point 3 of the model flat-rate agreement. L’objectif principal du projet est de sensibiliser les 2 350 habitants de Radom et la municipalité de Jedlnia-Letnisko et Gózd (dont 1.175 M et 1.175 K) aux risques du diabète sucré et à être couverts par des consultations médicales pour la détection du diabète d’ici au 30 août 2019. Les tâches prévues sont conformes aux objectifs du programme régional de santé du gouvernement régional de voïvodie de Mazowieckie (RPZ) et comprennent: action d’information (Module I), consultations médicales et activités éducatives (Module II) et formation du personnel médical (Module III). Les principales PARTIES DU PROJET couverts par un soutien direct sont les personnes de la région. 60 ans vivant à Radom et dans la municipalité de Jedlnia-Latnisko et Gózd, non encore diagnostiqués dans le sens du diabète, qui signeront une copie du consentement éclairé à participer au projet. En outre, les médecins/infirmières et le personnel des établissements médicaux seront également ciblés. INDICATEUR DE RÉSULTAT ATTENDU: nombre de prestataires de services de santé post-projet bénéficiant d’un soutien dans le cadre du programme. INDICATEURS DE RÉALISATION ESCOMPTÉS: le nombre de personnes exposées au risque de pauvreté ou d’exclusion sociale couvertes par les services de santé dans le cadre du programme, le nombre de membres du personnel médical participant à la formation et le nombre de conférences à l’intention des patients sur l’alimentation et l’activité physique en particulier. Les autres indicateurs spécifiés dans le RP seront indiqués et communiqués conformément à l’article 30, paragraphe 3, du modèle de contrat forfaitaire pour le concours en question. Hauptzweck des Projekts ist es, die 2350 Einwohner von Radom und der Gemeinde Jedlnia-Letnisko und Gózd (einschließlich 1.175 M und 1.175 K) für die Risiken von Diabetes mellitus zu sensibilisieren und bis zum 30.08.2019 medizinische Konsultationen zum Nachweis von Diabetes durchzuführen. Die geplanten Aufgaben stehen im Einklang mit den Zielen des regionalen Gesundheitsprogramms der Woiwodschaft Mazowieckie (RPZ) und umfassen: Informationsmaßnahme (Modul I), medizinische Beratung und Bildungsaktivitäten (Modul II) und Ausbildung des medizinischen Personals (Modul III). Die wichtigsten PARTIEN DES PROJEKTs, die durch direkte Unterstützung abgedeckt werden, sind Menschen in der Region. 60 Jahre alt, die in Radom und der Gemeinde Jedlnia-Latnisko und Gózd leben, noch nicht diagnostiziert in Richtung Diabetes, die einen Druck der Einwilligung nach Aufklärung zur Teilnahme an dem Projekt unterzeichnen wird. Darüber hinaus werden auch Ärzte/Krankenschwestern und Mitarbeiter medizinischer Einrichtungen angesprochen. ERWARTETER ERGEBNISINDIKATOR: Zahl der im Rahmen des Programms unterstützten Gesundheitsdienstleister nach dem Projekt. ERWARTETE OUTPUTINDIKATOREN: die Zahl der von Armut oder sozialer Ausgrenzung bedrohten Personen, die im Rahmen des Programms von Gesundheitsdiensten abgedeckt sind, die Zahl der an der Ausbildung teilnehmenden medizinischen Mitarbeiter und die Zahl der Vorträge für Patienten über Ernährung und körperliche Aktivität im Besonderen. Die anderen im RP genannten Indikatoren werden gemäß § 30 Abs. 3 des Muster-Pauschalvertrags für den betreffenden Wettbewerb ausgewiesen und gemeldet. Im Ergebnisindikator Nr. 2 und den Outputindikatoren Nrn. 3, 4, 5, 6, 7 und 8 hat der Antragsteller die Zielwerte auf der Ebene von „0“ angegeben – ihre Schätzung ist nur im Stadium der Projektdurchführung möglich und wird gemäß den Bestimmungen der Subventionsvereinbarung gemeldet – die Informationen werden innerhalb der in § 30 Abs. 3 der Musterpauschale festgelegten Frist übermittelt. Het belangrijkste doel van het project is de 2,350 inwoners van Radom en de gemeente Jedlnia-Letnisko en Gózd (waaronder 1,175 M en 1.175 K) bewust te maken van de risico’s van diabetes mellitus en uiterlijk op 30 augustus 2019 te worden behandeld door medisch overleg voor de opsporing van diabetes. De geplande taken zijn in overeenstemming met de doelstellingen van het regionale gezondheidsprogramma van de regionale overheid van het woiwodschap Mazowieckie (RPZ) en omvatten: voorlichtingsactie (module I), medische consultaties en educatieve activiteiten (module II) en opleiding van medisch personeel (module III). De belangrijkste PARTIJEN VAN HET PROJECT die onder directe steun vallen, zijn de mensen in de regio. 60 jaar oud wonend in Radom en de gemeente Jedlnia-Latnisko en Gózd, nog niet gediagnosticeerd in de richting van diabetes, die een afdruk van geïnformeerde toestemming om deel te nemen aan het project zal ondertekenen. Daarnaast zullen ook artsen/verpleegkundigen, personeel van medische voorzieningen worden ingezet. VERWACHTE RESULTAATINDICATOR: aantal in het programma ondersteunde aanbieders van gezondheidsdiensten na het project. VERWACHTE OUTPUTINDICATOREN: het aantal mensen dat risico loopt op armoede of sociale uitsluiting dat onder de gezondheidsdiensten van het programma valt, het aantal medisch personeel dat aan de opleiding deelneemt en het aantal lezingen voor patiënten over voeding en lichaamsbeweging in het bijzonder. De overige in het RP gespecificeerde indicatoren zullen worden getoond en gerapporteerd overeenkomstig § 30, lid 3, van het forfaitaire modelcontract voor de betrokken mededinging. In de resultaatindicator nr. 2 en outputindicatoren nrs. 3, 4, 5, 6, 7 en 8 gaf de aanvrager de streefwaarden aan op het niveau van „0” — hun schatting is alleen mogelijk in het stadium van de uitvoering van het project en zal worden gerapporteerd overeenkomstig de bepalingen van de subsidieovereenkomst — de informatie zal worden verstrekt binnen de in §30, lid 3, van de modelforfaitaire overeenkomst vastgestelde termijn. L'obiettivo principale del progetto è quello di sensibilizzare i 2.350 abitanti di Radom e del comune di Jedlnia-Letnisko e Gózd (compresi 1.175 M e 1.175 K) in merito ai rischi di diabete mellito e di essere oggetto di consultazioni mediche per l'individuazione del diabete entro il 30.8.2019. I compiti previsti sono in linea con gli obiettivi del programma regionale per la salute del governo regionale del voivodato di Mazowieckie (RPZ) e comprendono: azione d'informazione (modulo I), consultazioni mediche e attività educative (modulo II) e formazione del personale medico (modulo III). Le principali PARTI DEL PROGETTO coperte dal sostegno diretto sono le persone della regione. 60 anni che vivono a Radom e nel comune di Jedlnia-Latnisko e Gózd, non ancora diagnosticati in direzione del diabete, che firmeranno una stampa del consenso informato a partecipare al progetto. Saranno inoltre presi di mira medici/infermieri, personale delle strutture mediche. INDICATORE DI RISULTATO PREVISTO: numero di fornitori di servizi sanitari post-progetto sostenuti nel programma.
