Grupę docelową stanowi 60 os.(36K,24M)osoby bezrobotne to jest 42 osoby i osoby bierne zawodowe to jest 18 osób zagrożonych ubóstwem lub wykluczeniem społecznym, doświadczających wielokrotnego wykluczenia społecznego rozumianego jako wykluczenie z powodu więcej niż 1 z przesłanek tj.: •osoby korzystające ze świadczeń z pomocy społecznej zgodnie z ustawą z dnia 12.03.2004r.o pomocy społecznej lub kwalifikujące się do korzystania ze świadczeń z pomocy społecznej to jest spełniające co najmniej jedną z przesłanek określonych w artykule 7 ustawy z dnia 12.03.2004r., •osoby korzystające z Programu Operacyjnego Pomoc Żywnościowa, •osoby zakwalifikowane do III profilu pomocy, zgodnie z ustawą z dnia 20.04.2004r.o promocji zatrudnienia i instytucjach rynku pracy, nie więcej niż 20% Uczestników Projektu to jest 12 osób, •osoby z niepełnosprawnością- minimum 30%Uczestników Projektu to jest 18 osób, zamieszkujących w rozumieniu Kodeksu Cywilnego na terenie woj.mazowieckiego w gminach poniżej progu defaworyzacji określonego w Mazowieckim barometrze ubóstwa i wykluczenia społecznego opisanych w pkt C.1.7 wniosku. Celem głównym projektu jest podwyższenie aktywności społeczno-zawodowej wyżej wymienionej grupy docelowej poprzez realizację zadań:(diagnozy indywidualnych potrzeb i potencjałów UP,poradnictwa psychologicznego, pośrednictwo pracy, treningu skutecznej komunikacji, warsztatów rozwoju osobistego,szkoleń i staży) od 01.08.2018r. do 31.07.2019r. prowadzącą do zatrudnienia minimum 18 osób oraz postępu w aktywizacji społecznej minimum 21 osób. W projekcie zostały opracowane następujące rezultaty: Liczba osób zagrożonych ubóstwem lub wykluczeniem społecznym, które uzyskały kwalifikacje po opuszczeniu programu-14 osób. -Liczba osób zagrożonych ubóstwem lub wykluczeniem społecznym poszukujących pracy po opuszczeniu programu-34 osoby. The target group consists of 60 people (36K,24M) unemployed persons, i.e. 42 persons and inactive persons, i.e. 18 people at risk of poverty or social exclusion, experiencing multiple social exclusion understood as exclusion due to more than 1 of the indications: • Persons benefiting from social assistance benefits in accordance with the Act of 12.03.2004r.o. on social assistance or eligible for social assistance benefits is meeting at least one of the conditions specified in Article 7 of the Act of 12.03.2004, • persons benefiting from the Operational Programme Food Aid, • persons qualified to the third profile of the aid, according to the Law of 20.04.2004r on promotion of employment and labour market institutions, is not more than 20 % of the participants in the project. The main objective of the project is to increase the socio-occupational activity of the above-mentioned target group through the implementation of tasks: (diagnosis of individual needs and potentials of the University, psychological advice, job mediation, effective communication training, personal development workshops, trainings and traineeships) from 01.08.2018 to 31.07.2019, leading to employment of at least 18 people and progress in social activation of at least 21 people. The project produced the following results: Number of people at risk of poverty or social exclusion who have been qualified after leaving programme-14. —Number of people at risk of poverty or social exclusion seeking work after leaving programme-34. Le groupe cible est de 60 personnes (36K,24 millions) au chômage, c’est-à-dire 42 personnes et inactives, soit 18 personnes exposées au risque de pauvreté ou d’exclusion sociale, confrontées à une exclusion sociale multiple, comprise comme une exclusion due à plus d’une des conditions, à savoir: •les bénéficiaires de prestations d’assistance sociale conformément à la loi du 12.03.2004 sur l’assistance sociale ou ayant droit aux prestations d’assistance sociale sont qu’ils remplissent au moins l’une des conditions prévues à l’article 7 de la loi du 12.03.2004, •les bénéficiaires du programme opérationnel d’aide alimentaire, les personnes qualifiées du troisième profil d’aide, conformément à la loi du 20.04.2004 sur la promotion de l’emploi et des institutions du marché du travail, 20 % au maximum des participants au projet sont 12 personnes, •les personnes handicapées — au moins 30 % des participants au projet sont 18 personnes au sens du Code civil au sens du Code civil. L’objectif principal du projet est d’accroître l’activité socioprofessionnelle du groupe cible susmentionné grâce à la mise en œuvre de tâches (diagnostic des besoins et des potentiels individuels de l’UP, conseil psychologique, médiation professionnelle, formation à la communication efficace, ateliers de développement personnel, formations et stages) du 01.08.2018 au 31.7.2019. conduisant à l’emploi d’un minimum de 18 personnes et à progresser dans l’activation sociale d’un minimum de 21 personnes. Les résultats suivants ont été élaborés dans le cadre du projet: Nombre de personnes exposées au risque de pauvreté ou d’exclusion sociale ayant obtenu des qualifications après avoir quitté le programme-14. —Nombre de personnes menacées de pauvreté ou d’exclusion sociale à la recherche d’un emploi après avoir quitté le programme-34. Zielgruppe sind 60 Personen (36K,24M) Arbeitslose, d. h. 42 Personen und Nichterwerbstätige, d. h. 18 von Armut oder sozialer Ausgrenzung bedrohte Personen, die mehrfach sozialer Ausgrenzung ausgesetzt sind, verstanden als Ausgrenzung aufgrund von mehr als einer der Bedingungen, d. h.: • Personen, die Sozialhilfeleistungen gemäß dem Gesetz vom 12.03.2004 über Sozialhilfe erhalten oder Sozialhilfeleistungen erhalten, besteht darin, dass sie mindestens eine der in Artikel 7 des Gesetzes vom 12.03.2004 festgelegten Voraussetzungen erfüllen, • Personen, die im Rahmen des operationellen Programms Nahrungsmittelhilfe in Anspruch genommen werden, • Personen, die gemäß dem Gesetz vom 20.4.2004 über die Förderung der Beschäftigung und der Arbeitsmarkteinrichtungen für das dritte Hilfsprofil qualifiziert sind, höchstens 20 % der Projektteilnehmer sind 12 Personen, • Personen mit Behinderungen – mindestens 30 % der Projektteilnehmer sind 18 Personen, die im Sinne des Zivilgesetzbuchs im Sinne des Zivilgesetzbuchs leben. Hauptziel des Projekts ist es, die sozioprofessionelle Aktivität der oben genannten Zielgruppe durch die Umsetzung von Aufgaben zu verstärken: (Diagnose der individuellen Bedürfnisse und Potenziale von UP, psychologische Beratung, Jobvermittlung, effektive Kommunikationsausbildung, persönliche Entwicklungsworkshops, Schulungen und Praktika) vom 01.08.2018 bis 31.07.2019. Im Projekt wurden folgende Ergebnisse entwickelt: Zahl der von Armut oder sozialer Ausgrenzung bedrohten Personen, die nach dem Ausscheiden aus dem Programm-14 Qualifikationen erworben haben. —Anzahl der von Armut oder sozialer Ausgrenzung bedrohten Personen, die nach dem Ausscheiden aus dem Programm einen Arbeitsplatz suchen-34. De doelgroep is 60 werklozen (36K,24 miljoen), d.w.z. 42 mensen en inactieven, d.w.z. 18 personen die het risico lopen op armoede of sociale uitsluiting, die te maken hebben met meervoudige sociale uitsluiting, begrepen als uitsluiting als gevolg van meer dan 1 van de voorwaarden, d.w.z.: •personen die sociale bijstandsuitkeringen ontvangen overeenkomstig de Wet van 12.3.2004 inzake sociale bijstand of in aanmerking komen voor sociale bijstandsuitkeringen, zijn dat zij voldoen aan ten minste één van de voorwaarden van artikel 7 van de wet van 12.3.2004, •personen die in aanmerking komen voor het operationele programma Voedselhulp, •personen die in aanmerking komen voor het derde steunprofiel, overeenkomstig de Wet van 20.4.2004 inzake de bevordering van werkgelegenheids- en arbeidsmarktinstellingen, niet meer dan 20 % van de deelnemers aan het project zijn 12 personen, •personen met een handicap — minimum 30 % van de deelnemers aan het project is 18 personen die in de zin van de Burgerlijk Wetboek leven. De belangrijkste doelstelling van het project is het vergroten van de sociaal-professionele activiteit van de bovengenoemde doelgroep door de uitvoering van taken: (diagnose van individuele behoeften en mogelijkheden van UP, psychologische begeleiding, werkbemiddeling, effectieve communicatietraining, workshops voor persoonlijke ontwikkeling, trainingen en stages) van 01.08.2018 tot 31.7.2019. Dit leidt tot een baan van minimaal 18 personen en vooruitgang bij sociale activering van minimaal 21 personen. In het project zijn de volgende resultaten ontwikkeld: Aantal personen dat met armoede of sociale uitsluiting wordt bedreigd en dat na het verlaten van het programma een diploma heeft behaald-14. —Aantal mensen dat risico loopt op armoede of sociale uitsluiting op zoek naar een baan na het verlaten van het programma-34. Il gruppo destinatario è di 60 persone (36K,24 milioni di disoccupati), vale a dire 42 persone e persone inattive, vale a dire 18 persone a rischio di povertà o di esclusione sociale, che subiscono un'esclusione sociale multipla, intesa come esclusione dovuta a più di una delle condizioni, ossia: •le persone che beneficiano di prestazioni di assistenza sociale ai sensi della legge del 12.3.2004 sull'assistenza sociale o che possono beneficiare di prestazioni di assistenza sociale sono quelle che soddisfano almeno una delle condizioni di cui all'articolo 7 della legge del 12.3.2004, • le persone che beneficiano del programma operativo di aiuto alimentare, • le persone qualificate al terzo profilo di aiuto, conformemente alla legge del 20.4.2004 sulla promozione dell'occupazione e delle istituzioni del mercato del lavoro, non più del 20 % dei partecipanti al progetto sono 12 persone, • persone con disabilità — almeno il 30 % dei partecipanti al progetto è 18 persone che vivono ai sensi del codice civile ai sensi del codice civile. L'obiettivo principale del progetto è quello di incrementare l'attività socio-professionale del suddetto gruppo target attraverso l'attuazione di compiti: (diagnostica di esigenze e potenzialità individuali di UP, consulenza psicologica, mediazione professionale, formazione efficace della comunicazione, workshop di sviluppo personale, formazioni e stage) dall'1.8.2018 al 31.7.2019. portando a un impiego di almeno 18 persone e a progressi nell'attivazione sociale di un minimo di 21 persone. Nel progetto sono stati sviluppati i seguenti risultati: Numero di persone a rischio di povertà o di esclusione sociale che hanno ottenuto qualifiche dopo aver lasciato il programma-14. —Numero di persone a rischio di povertà o di esclusione sociale in cerca di lavoro dopo aver lasciato il programma-34.
