P.,,Droga do Samodzielności" będzie realizowany na terenie powiatu żagańskiego w okresie 01.07.2019-31.12.2021, przez Powiatowe Centrum Pomocy Rodzinie w Żaganiu w partnerstwie z: Fundacją Eudajmonia (partner1 FEU)-odp.za aktywizację społ.ON) i Lutolską Spółdzielnią Socjalną WINDA (partner2 LSS) -odp.za aktywizację społ.osób z pieczy i aktywizację zaw. osób z pieczy i ON.Skierowany będzie do co najmniej 200 osób, w tym co najmniej 35 os.z niepełnosprawnościami,zagrożonych ubóstwem i wykluczeniem społecznym, należących do gr. docelowych m.in.: osób niepełnosprawnych, osób w pieczy zastępczej, osób opuszczających pieczę zastępczą, rodzin przeżywających trudności w pełnieniu funkcji opiekuńczo-wychowawczych i środowiska/otoczenie tych grup. Główny cel projektu-zwiększenie szans na zatrudnienie w/w osób poprzez angażowanie ich w działania reint. społeczno-zawodowej przy zastosowaniu usł. aktywnej integracji o charakterze: społecznym, zawodowym, edukacyjnym (zgodnie z Wytycznymi w zakresie realizacji i przedsięwzięć w obszarze włączenia społecznego i zwalczania ubóstwa z wykorzystaniem środków EFS i EFRR na lata 2014-2020). P. «Route vers l’indépendance» sera mis en œuvre dans le district de ▲agań au cours de la période allant du 1er juillet 2019 au 31.12.2021, par le Centre de district d’aide à la famille de Şagań, en partenariat avec: La Fondation Eudajmonia (partenaire FEU1) — responsable de l’activation de la société et de la coopérative sociale lutolienne winda (partenaire 2 de la LSS) — pour l’activation des personnes avec soin et activation des personnes ayant la garde et l’ON.Il sera destiné à au moins 200 personnes, dont au moins 35 personnes handicapées, qui sont exposées au risque de pauvreté et d’exclusion sociale, appartenant aux cibles cibles, entre autres: les personnes handicapées, les personnes placées en famille d’accueil, les personnes qui quittent la famille d’accueil, les familles qui éprouvent des difficultés à s’acquitter des fonctions de prise en charge et d’éducation et l’environnement/la situation de ces groupes. L’objectif principal du projet est d’accroître l’employabilité des personnes susmentionnées en les associant à des activités de réintroduction socioprofessionnelle utilisant des services d’inclusion active: social, professionnel et éducatif (conformément aux lignes directrices pour la mise en œuvre et les actions dans le domaine de l’inclusion sociale et de la lutte contre la pauvreté au moyen des ressources du FSE et du FEDER pour la période 2014-2020). P. „Road to Independentness“ wird im Bezirk Żagań in der Zeit vom 01.07.2019-31.12.2021 vom Kreiszentrum für Familienhilfe in Żagań in Zusammenarbeit mit: Die Eudajmonia Foundation (FEU Partner1) – verantwortlich für die Aktivierung der Gesellschaft und der Lutolian Social Cooperative winda (Partner2 der LSS) – für die Aktivierung von Pflege- und Aktivierungspersonen mit Haft und ON.Sie richtet sich an mindestens 200 Menschen, darunter mindestens 35 Menschen mit Behinderungen, die von Armut und sozialer Ausgrenzung bedroht sind, die unter anderem zu den Zielen gehören: Menschen mit Behinderungen, Pflegebedürftige, Personen, die Pflegeleistungen verlassen, Familien, die Schwierigkeiten bei der Wahrnehmung von Betreuungs- und Bildungsfunktionen haben, sowie die Umwelt/Umstände dieser Gruppen. Hauptziel des Projekts ist es, die Beschäftigungsfähigkeit der oben genannten Personen zu verbessern, indem sie in sozial-berufliche Wiedereinführungsmaßnahmen einbezogen werden, die aktive Eingliederungsdienste nutzen: soziale, berufliche und bildungspolitische Maßnahmen (gemäß den Leitlinien für die Umsetzung und Maßnahmen im Bereich der sozialen Eingliederung und der Armutsbekämpfung unter Verwendung von ESF- und EFRE-Mitteln für den Zeitraum 2014-2020). P. „Weg naar onafhankelijkheid” zal in de periode 1.7.2019-31.12.2021 in het district Șagań worden uitgevoerd door het District Centre for Family Assistance in Șagań, in samenwerking met: De Eudajmonia Foundation (FEU Partner1)-verantwoordelijk voor de activering van de samenleving en de Lutolian Social Cooperative winda (partner 2 van de LSS) — voor de activering van mensen met zorg en activering van personen met voogdij en ON.Het zal gericht zijn op ten minste 200 mensen, waaronder ten minste 35 mensen met een handicap, die het risico lopen op armoede en sociale uitsluiting, die tot de doelstellingen behoren, onder meer: personen met een handicap, personen in pleegzorg, personen die de pleegzorg verlaten, gezinnen die moeilijkheden ondervinden bij de uitvoering van zorg- en onderwijsfuncties, en het milieu/de ombuiging van deze groepen. De belangrijkste doelstelling van het project is de inzetbaarheid van de bovengenoemde personen te vergroten door hen te betrekken bij activiteiten op het gebied van sociaal-beroepsgerichte herintroductie waarbij gebruik wordt gemaakt van actieve inclusiediensten: sociaal, professioneel, educatief (overeenkomstig de richtsnoeren voor de uitvoering en acties op het gebied van sociale inclusie en armoedebestrijding met behulp van ESF- en EFRO-middelen voor 2014-2020). La "strada verso l'indipendenza" sarà attuata nel distretto di Żagań nel periodo 01.07.2019-31.12.2021 dal Centro distrettuale per l'assistenza familiare di Żagań in collaborazione con: La Fondazione Eudajmonia (FEU Partner1)-Responsabile per l'attivazione della società e la Lutolian Social Cooperative Winda (partner2 della LSS) — per l'attivazione di persone con cura e attivazione di persone con custodia e ON. Sarà rivolta ad almeno 200 persone, tra cui almeno 35 persone con disabilità, che sono a rischio di povertà ed esclusione sociale, appartenenti agli obiettivi target, tra cui: le persone con disabilità, le persone in affidamento, le persone che abbandonano l'affidamento, le famiglie che incontrano difficoltà nello svolgimento delle funzioni di assistenza e di istruzione e l'ambiente/circostanza di questi gruppi. L'obiettivo principale del progetto è quello di aumentare l'occupabilità delle suddette persone coinvolgendole in attività di reintroduzione socio-professionale utilizzando servizi di inclusione attiva: sociale, professionale, educativo (conformemente agli orientamenti per l'attuazione e le azioni nel settore dell'inclusione sociale e della lotta contro la povertà utilizzando le risorse del FSE e del FESR per il periodo 2014-2020).
