Celem głównym projektu jest "Poprawa stanu zdrowia 440 dzieci (242 uczennic i 198 uczniów) ze zdiagnozowaną otyłością uczęszczających do klas I-VI ze SP w W-wie i Nowym Dworze Mazowieckim (dalej NDM), w zakresie otyłości poprzez udział w interwencji terapeutycznej zaplanowanej w ramach proj. do 30.07.2021r.". Planowane zadania są zgodne z założeniami Regionalnego programu zdrowotnego w zakresie chorób kręgosłupa i otyłości wśród dzieci z województwa mazowieckiego i obejmują: Moduł dotyczący OTYŁOŚCI. W ramach projektu zaplanowano realizację następujących zadań: 1.Działania informacyjno-promocyjne połączone z kwalifikacją uczestników; 2.Działania informacyjno-edukacyjne grup objętych wsparciem w ramach modułu dotyczącego otyłości; 3.Interwencja terapeutyczna – badania; 4.Interwencja terapeutyczna - porady dietetyczne i zajęcia ruchowe; 5.Działania informacyjno-szkoleniowe dla personelu medycznego oraz personelu szkół; 6.Działania informacyjno-edukacyjne dla osób z otoczenia dzieci uczestniczących w programie. GŁÓWNI UCZESTNICY PROJEKTU objęci wsparciem bezpośrednim to dzieci uczęszczające do klas I-VI szkół podstawowych województwa mazowieckiego ze stwierdzoną na podstawie dokumentacji medycznej lub kwalifikacji przeprowadzonej przez beneficjenta OTYŁOŚCIĄ (uczęszczające do SP w W-wie i NDM). Ponadto działaniami zostaną objęci również: personel medyczny, otoczenie, w tym nauczyciele i opiekunowie prawni. ZAKŁADANY WSKAŹNIK REZULTATU to m.in.: liczba wspartych w programie miejsc świadczenia usług zdrowotnych, istniejących po zakończeniu projektu (dodatkowe wskazano w sekcji C). ZAKŁADANY WSKAŹNIK PRODUKTU to m.in: liczba osób zagrożonych ubóstwem lub wykluczeniem społecznym objętych usługami zdrowotnymi w programie (dodatkowe wskazano w sekcji C). The main objective of the project is “Improving the health of 440 children (242 students and 198 students) with diagnosed obesity attending classes I-VI from the SP in W-wa and Nowy Dwór Mazowiecki (hereinafter referred to as NDM), in terms of obesity through participation in therapeutic intervention planned under the project until 30.07.2021”. The planned tasks are consistent with the objectives of the Regional Health Programme in the field of vertebral diseases and obesity among children from Mazowieckie voivodship and include: Obesity module. The project plans to carry out the following tasks: 1.Information and promotion activities combined with the qualification of participants; 2.Information and education activities of the supported groups under the obesity module; 3.Tererapeutic intervention – research; 4.Tererapeutic intervention – dietary advice and motor activities; 5.Information and training activities for medical staff and school staff; 6.Information and education activities for people from around children participating in the programme. The main participants of the project included in direct support are children attending classes I-VI of primary schools of Mazowieckie voivodship with established on the basis of medical records or qualifications carried out by the beneficiary with obesity (employment to the SP in W-wa and NDM). In addition, the activities will also include: medical staff, environment, including teachers and legal guardians. The assumed RESULT CONDITIONER is among others: number of post-project health services supported in the programme (additional as indicated in section C). The assumed PRODUCT Indicators include: number of people at risk of poverty or social exclusion covered by health services in the programme (additional as indicated in section C). Other indicators specified in RPZ will be shown and reported in accordance with §34 point 3 of the standard contract for the contest. The project will be fulfilled by the MERYTOR CRITERIUM. L’objectif principal du projet est «Améliorer la santé de 440 enfants (242 élèves et 198 élèves) ayant reçu un diagnostic d’obésité dans les classes I-VI du SP de W-w et Nowy Dwór Mazowiecki (ci-après dénommé NDM), dans le domaine de l’obésité, en participant à l’intervention thérapeutique prévue dans le cadre du projet jusqu’au 30.07.2021». Les tâches prévues sont conformes aux objectifs du programme régional de santé pour les maladies de la colonne vertébrale et l’obésité chez les enfants de la voïvodie de Mazowieckie et comprennent: Module sur l’OTILITÉ. Dans le cadre du projet, les tâches suivantes sont prévues: 1.Les activités d’information et de promotion combinées à la qualification des participants; 2.Les activités d’information et d’éducation des groupes soutenus dans le cadre du module sur l’obésité; 3. Intervention thérapeutique — recherche; 4. Intervention thérapeutique — conseils diététiques et classes motrices; 5. Activités d’information et de formation à l’intention du personnel médical et scolaire; 6. Activités d’information et d’éducation à l’intention des personnes des environs des enfants participant au programme. Les principales PARTIES DU PROJET bénéficiant d’une aide directe sont les enfants qui fréquentent les classes I à VI des écoles primaires de la voïvodie de Mazowieckie et qui sont établis sur la base de documents médicaux ou de qualifications effectuées par le bénéficiaire OTYŁOŚCIA (participant au SP dans W-wo et NDM). En outre, les activités suivantes seront également couvertes: personnel médical, environnement, y compris les enseignants et les tuteurs légaux. Le RESULTATIQUE présumé est, entre autres: nombre de prestataires de services de santé bénéficiant d’un soutien dans le cadre du programme, existant après la fin du projet (les autres sont indiqués à la section C). Les PRODUITS PRODUITs présumés comprennent: nombre de personnes exposées au risque de pauvreté ou d’exclusion sociale couvertes par les services de santé dans le cadre du programme (supplément est indiqué à la section C). Les autres indicateurs spécifiés dans le RP seront affichés et communiqués conformément à l’article 34, paragraphe 3, du modèle de contrat type pour le concours en question. Das Hauptziel des Projekts ist "Verbesserung der Gesundheit von 440 Kindern (242 Studenten und 198 Studenten), die an den Klassen I-VI des SP in W-w und Nowy Dwór Mazowiecki (nachfolgend NDM) im Bereich Fettleibigkeit durch Teilnahme an der im Rahmen des Projekts geplanten therapeutischen Intervention bis 30.07.2021 diagnostiziert wurden. Die geplanten Aufgaben stehen im Einklang mit den Zielen des Regionalen Gesundheitsprogramms für Wirbelsäulenkrankheiten und Adipositas bei Kindern aus der Woiwodschaft Mazowieckie und umfassen: Modul zur OTILITÄT. Im Rahmen des Projekts sind folgende Aufgaben geplant: 1.Informations- und Absatzförderungsmaßnahmen in Verbindung mit der Qualifikation der Teilnehmer; 2.Informations- und Bildungsaktivitäten der im Rahmen des Adipositas-Moduls unterstützten Gruppen; 3.Therapeutische Intervention – Forschung; 4.Therapeutische Intervention – Ernährungsberatung und Motorklassen; 5.Informations- und Schulungsmaßnahmen für medizinisches Personal und Schulpersonal; 6.Informations- und Bildungsaktivitäten für Menschen aus der Umgebung von Kindern, die an dem Programm teilnehmen. Die wichtigsten, durch direkte Unterstützung abgedeckten PROJEKTE sind Kinder, die die Klassen I bis VI der Grundschulen der Woiwodschaft Mazowieckie besuchen und auf der Grundlage medizinischer Unterlagen oder Qualifikationen des Begünstigten OTYŁOŚCIA (Teilnahme an SP in W-wo und NDM) niedergelassen sind. Darüber hinaus werden folgende Aktivitäten abgedeckt: medizinisches Personal, Umgebung, einschließlich Lehrern und Erziehungsberechtigten. Die angenommene RESULTATIC ist unter anderem: Zahl der im Rahmen des Programms unterstützten Gesundheitsdienstleister, die nach dem Ende des Projekts bestehen (zusätzliche Anbieter sind in Abschnitt C angegeben). Die angenommenen PRODUKT-PRODUKTE umfassen: Zahl der von Armut oder sozialer Ausgrenzung bedrohten Personen, die im Rahmen des Programms von Gesundheitsdiensten abgedeckt werden (zusätzlich ist in Abschnitt C angegeben). Die anderen im RP genannten Indikatoren werden gemäß § 34 Abs. 3 des Mustervertragsmodells für den betreffenden Wettbewerb ausgewiesen und gemeldet. De belangrijkste doelstelling van het project is "Verbetering van de gezondheid van 440 kinderen (242 studenten en 198 studenten) met obesitas in de klassen I-VI van de SP in W-w en Nowy Dwór Mazowiecki (hierna "NDM" genoemd), op het gebied van obesitas door deel te nemen aan de therapeutische interventie die in het kader van het project is gepland tot 30.7.2021. De geplande taken zijn in overeenstemming met de doelstellingen van het regionale gezondheidsprogramma voor wervelkolomziekten en obesitas bij kinderen uit het woiwodschap Mazowieckie en omvatten: Module over OTILITEIT. In het kader van het project zijn de volgende taken gepland: 1.Informatie- en promotieactiviteiten in combinatie met de kwalificatie van deelnemers; 2.Informatie- en onderwijsactiviteiten van de groepen die worden ondersteund in het kader van de module obesitas; 3.Therapeutische interventie — onderzoek; 4.Therapeutic interventie — dieetadvies en motorklassen; 5.Informatie- en opleidingsactiviteiten voor medisch en schoolpersoneel; 6.Informatie en educatieve activiteiten voor mensen uit de omgeving van kinderen die deelnemen aan het programma. De belangrijkste PARTIJEN VAN HET PROJECT die onder rechtstreekse steun vallen, zijn kinderen die de klassen I tot en met VI van de basisscholen van het woiwodschap Mazowieckie volgen en zijn vastgesteld op basis van medische documentatie of kwalificaties die door de begunstigde OTYŁOŚCIA (deelnemend aan SP in W-wo en NDM) worden uitgevoerd. Daarnaast zullen ook de volgende activiteiten worden bestreken: medisch personeel, omgeving, met inbegrip van leraren en wettelijke voogden. De veronderstelde RESULTATIC is onder meer: aantal aanbieders van gezondheidsdiensten dat in het programma wordt ondersteund, dat na afloop van het project bestaat (aanvullende dienstverleners zijn vermeld in deel C). De veronderstelde PRODUCTPRODUCTEN omvatten: aantal personen dat risico loopt op armoede of sociale uitsluiting dat onder de gezondheidsdiensten van het programma valt (aanvullend wordt vermeld in deel C). De overige in de RP gespecificeerde indicatoren zullen worden getoond en gerapporteerd overeenkomstig § 34, lid 3, van het model van het standaardcontract voor de betrokken mededinging.
