Projekt pt.: „Poprawa jakości i dostępności świadczonych usług medycznych i diagnostycznych oraz wsparcie opieki koordynowanej przez SP ZOZ PIASTUN w Piastowie” realizowany będzie przez Miasto Piastów. Użytkownikiem / Operatorem powstałej infrastruktury i realizatorem działań będzie: Samodzielny Publiczny Zakład Opieki Zdrowotnej PIASTUN w Piastowie, który jako jednostka organizacyjna realizuje zadania własne Gminy Miasto Piastów z zakresu ochrony zdrowia. CELEM GŁÓWNYM PROJEKTU jest: „Poprawa jakości i dostępności świadczonych usług medycznych i diagnostycznych oraz wsparcie opieki koordynowanej przez SP ZOZ PIASTUN w Piastowie”. ZAKRES PROJEKTU obejmuje następujące zadania: Zadanie nr 1: Zakup sprzętu medycznego dla SP ZOZ PIASTUN w Piastowie; Zadanie nr 2: Zakup radiologicznego systemu informatycznego klasy RIS/PACS/WEB wraz ze stacją lekarską i duplikatorem; Zadanie nr 3: Integracja urządzeń z dziedzinowym systemem HIS; Zadanie nr 4: Wymagane licencje; Zadanie nr 5: Prace adaptacyjno – montażowe; Zadanie nr 6: Promocja projektu. GRUPY DOCELOWE: - Samodzielny Publiczny Zakład Opieki Zdrowotnej PIASTUN w Piastowie – podmiot prowadzący działalność leczniczą, funkcjonujący w publicznym systemie ochrony zdrowia, - pacjenci – obawiający się o swoje zdrowie i oczekujący fachowej oraz sprawnej opieki medycznej w zakresie profilaktyki i leczenia chorób, które są istotną przyczyną dezaktywizacji zawodowej; - mieszkańcy Piastowa, powiatu pruszkowskiego i regionu, którym zależy na szybkim dostępie do nowoczesnej i profesjonalnej opieki medycznej w zakresie chorób, które są istotną przyczyną dezaktywizacji zawodowej; - społeczeństwo miasta Piastów, powiatu pruszkowskiego, województwa mazowieckiego, którego jakość życia ulegnie poprawie poprzez zapewnienie wyższego standardu opieki medycznej. Projekt jest zgodny z MAPAMI POTRZEB ZDROWOTNYCH dla województwa mazowieckiego przyjętymi przez Ministerstwo Zdrowia i obowiązującymi na dzień ogłoszenia konkursu. (Polish)

Project titled: “Improving the quality and availability of medical and diagnostic services and support of care coordinated by the ZOZ piastun in Piastów” will be implemented by the City of Piastów. The user/operator of the created infrastructure and the executioner of activities will be: Independent Public Health Care Department in Piastów, which as an organizational unit carries out its own tasks of the City of Piastów in the field of health care. The main objective of the project is to: “Improving the quality and availability of medical and diagnostic services and support for integrated care by the SP ZOZ piastun in Piastów”. The scope of the project shall cover the following tasks: Task 1: Purchase of medical equipment for SP ZOZ huastun in Piastów; Task 2: Purchase of a radiological RIS/PACS/WEB IT system with a medical station and a duplicator; Task 3: Integration of equipment into the domain HIS; Task 4: The required licences; Task 5: Adaptive and assembly work; Task 6: Promotion of the project. TARGET GROUPS: — Independent Public Health Care Department in Piastów – entity conducting medical activities, operating in the public health system, – patients – fearing for their health and expecting professional and efficient medical care in the field of prevention and treatment of diseases which are a significant cause of professional inactivation; — residents of Piastów, the Prussian poviat and the region who want to quickly access modern and professional medical care in the field of diseases which are a significant cause of professional inactivation; — the society of the city of Piastów, the Prussian county and Mazovia voivodship, whose quality of life will be improved by providing a higher standard of medical care. The project is compatible with maps of health care for Mazowieckie voivodship adopted by the Ministry of Health and in force on the day of the announcement of the competition. (English)

Projet: «Améliorer la qualité et la disponibilité des services médicaux et diagnostiques et des soins de soutien coordonnés par le SP ZOZ piastun à Piastów» sera mis en œuvre par la ville de Piastów. L’utilisateur/l’exploitant de l’infrastructure qui en résulte et la mise en œuvre des activités seront: Département de santé publique indépendant de Piastów, qui, en tant qu’unité organisationnelle, s’acquitte de ses propres tâches de la municipalité de Piastów dans le domaine de la protection de la santé. L’objectif principal du projet est de: «Amélioration de la qualité et de la disponibilité des services médicaux et de diagnostic et des soins de soutien coordonnés par le centre SP ZOZ à Piastów». Le champ d’application du PROJET couvre les tâches suivantes: Activité 1: Achat de matériel médical pour les hubs SP ZOZ à Piastów; Activité 2: Achat d’un système d’information radiologique RIS/PACS/WEB avec station médicale et doublon; Activité 3: Intégration de l’équipement dans le système HIS de terrain; Activité 4: Les licences requises; Tâche 5: Les travaux d’adaptation et d’assemblage; Activité 6: Promotion du projet. GROUPES CIBLES: — Centre de santé publique indépendant de Piastów — entité opérant dans le système de santé publique, patients — qui sont préoccupés par leur santé et attendent des soins médicaux professionnels et efficaces pour la prévention et le traitement des maladies qui sont une cause majeure de désactivation professionnelle; — les habitants de Piastów, du district prussien et de la région, qui veulent accéder rapidement à des soins médicaux modernes et professionnels dans le domaine des maladies, qui sont une cause importante d’inactivation professionnelle; — société de la ville de Piastów, le quartier prussien, la voïvodie de Mazowieckie, dont la qualité de vie sera améliorée en fournissant un niveau plus élevé de soins médicaux. Le projet est conforme aux cartes des besoins sanitaires pour la voïvodie de Mazowieckie adoptées par le ministère de la Santé et en vigueur le jour de l’annonce du concours. (French)

Der Nutzer/Betreiber der daraus resultierenden Infrastruktur und die Durchführung der Maßnahmen sind: Unabhängige Abteilung für öffentliche Gesundheitsversorgung Piastów, die als Organisationseinheit eigene Aufgaben der Gemeinde Piastów im Bereich des Gesundheitsschutzes wahrnimmt. Hauptziel des Projekts ist es, „Verbesserung der Qualität und Verfügbarkeit von medizinischen und diagnostischen Dienstleistungen und Unterstützung der Betreuung durch SP ZOZ-Hub in Piastów“. Der Anwendungsbereich des PROJECT umfasst folgende Aufgaben: Aktivität 1: Kauf von medizinischer Ausrüstung für SP ZOZ-Hubs in Piastów; Aktivität 2: Erwerb von RIS/PACS/WEB radiologischem Informationssystem mit medizinischer Station und Doppelfunktion; Aktivität 3: Integration von Geräten in das HIS-System; Aktivität 4: Erforderliche Lizenzen; Aufgabe 5: Anpassungs- und Montagearbeiten; Aktivität 6: Förderung des Projekts.
