Настоящият проект планира осигуряване на почасови мобилни интегрирани здравно-социални услуги за възрастни хора и лица с увреждания в техните домове. За реализация на дейността е създадено партньорство между общините Ботевград и Мирково, чрез подписано споразумение за сътрудничество, като се предвижда проекта да обхване най-малко 105 потребители. Всички дейности са съобразени с Плана за действие за периода 2018-2021 г. за изпълнение на Националната стратегия за дългосрочна грижа, като целта е подобряване на качеството на живот и възможностите за социално включване чрез осигуряване на грижи, предоставени от мултидисциплинарен екип, включващ различни специалисти. This project plans to provide hourly mobile integrated health-social services for elderly and disabled people in their homes. A partnership between the municipalities of Botevgrad and Mirkovo was established for the implementation of the activity through a signed cooperation agreement, with the project to cover at least 105 users. All activities are in line with the 2018-2021 Action Plan for the implementation of the National Strategy for Long-term Care, with the aim of improving the quality of life and opportunities for social inclusion by providing care provided by a multidisciplinary team involving different professionals. Ce projet prévoit de fournir des services sanitaires et sociaux intégrés et mobiles à l’intention des personnes âgées et handicapées dans leurs foyers. Un partenariat entre les municipalités de Botevgrad et Mirkovo a été établi pour la mise en œuvre de l’activité par le biais d’un accord de coopération signé, le projet couvrant au moins 105 utilisateurs. Toutes les activités sont conformes au plan d’action 2018-2021 pour la mise en œuvre de la stratégie nationale pour les soins de longue durée, dans le but d’améliorer la qualité de vie et les possibilités d’inclusion sociale en fournissant des soins fournis par une équipe pluridisciplinaire associant différents professionnels.
