Obszarem interwencji jest aktywizacja społeczno-zawodowa 50 os. (35ki15m) zamieszkałych na terenie gmin poniżej defowaryzacji wymienionych w mazowieckim barometrze ubóstwa i wykluczenia społ.(KMS2), które korzystają ze świadczeń z pomocy społecznej zgodnie z ustawą z dnia 12 marca2004 o pomocy społecznej lub kwalifikujące się do objęcia wsparciem pomocy społęcznej tj. spełniające co najmniej jedna z przesłanek określonych w art.7 tejże ustawy . Wsparcie tych osób odbywać się będzie na podstawie Ścieżki Reintegracji stworzonej Indywidualnie dla każdej osoby, z uwzględnieniem diagnozy sytuacji problemowej lub zagrożenia sytuacją problemową,zasobów,potencjału,predyspozycji, potrzeb. Z każdym ucz. zostanie podpisana umowa na wzór kontraktu socjalnego. (KD3). Działania przewidziane w projekcie są ukierunkowane na zwiększenie szans ucz. na zatrudnienie jak również są zapobiegawcze zjawisku wykluczenia społecznego i ubóstwa. W wyniku działań projektowych zostanie osiągnięty minimalny wskaźnik efektywności społecznej na poziomie 34% zaś w wskaźnik zatrudnieniowy będzie wynosił min. 22% (KD5). Projekt będzie realizowany w terminie od 01.04.2018-31.12.2019 (21miesięcy)-(KD1). Projekt jest realizowany w PARTNERSTWIE :PRZEDSIĘBIORSTWO PRYWATNE -Akademia Nauki w Płońsku (AKN), Centrum Edukacyjno-Doradcze –INSTYTUCJA RYNKU PRACY- WNIOSKODAWCA – (LID) (KMS3). W projekcie będą prowadzone działania na rzecz kobiet i mężczyzn i niedyskryminowanie poprzez równy dostęp do udziału w projekcie bez względu na płeć, wyznanie, stan zdrowia czy inne przesłanki powodujące dyskryminację. Projekt zakłada realizację następujących zadań: 1. Stworzenie indywidualnej ścieżki reintegracji,2. Podpisanie umów na wzór kontraktu socjalnego z każdym z uczestnikiem,3.Poradnictwo zawodowe,4.Kurs umożliwiający nabycie kwalifikacji komputerowych,5.Kursy umożliwiające nabycie nowych kwalifikacji zawodowych,6.Akademia Rodziny,7.Staże zawodowe. The area of intervention is socio-occupational activation of 50 people. (35ki15m) living in municipalities below defovarisation listed in the Mazovian poverty barometer and social exclusion (KMS2), which benefit from social assistance in accordance with the Social Assistance Act of 12 March2004 or eligible for social assistance, i.e. fulfilling at least one of the conditions set out in Article 7 of that Act. Support for these people will be based on the Reintegration Path created individually for each person, taking into account the diagnosis of a problem situation or a threat of problem situation, resources, potential, predisposition, needs. With each teacher, a contract will be signed on the basis of a social contract. (KD3). The actions envisaged in the project are aimed at increasing the employability of learners as well as preventive phenomena of social exclusion and poverty. As a result of the project activities, the minimum social efficiency ratio of 34 % will be achieved and the employment rate will be at least 22 % (KD5). The project will be implemented from 01.04.2018-31.12.2019 (21 months)-(KD1). The project is implemented in PARTNERSHIP: Privileged PRICES – Academy of Sciences in Płońsk (AKN), Education and Advisory Centre – Work Market institution – Applicant – (LID) (KMS3). The project will promote women and men and non-discrimination through equal access to participation in a project regardless of sex, religion, health or other grounds that lead to discrimination. Le domaine d’intervention est l’activation socioprofessionnelle de 50 personnes. (35ki15m) résidant dans des municipalités dont la défowarisation est mentionnée dans le Baromètre de la pauvreté et de l’exclusion sociale de Mazovian (KMS2), qui perçoivent des prestations d’assistance sociale conformément à la loi du 12 mars 2004 relative à l’assistance sociale ou qui peuvent bénéficier de l’aide sociale, c’est-à-dire qu’elles remplissent au moins l’une des conditions prévues à l’article 7 de cette loi. Le soutien de ces personnes sera basé sur le Chemin de Réintégration créé individuellement pour chaque personne, en tenant compte du diagnostic d’une situation problématique ou de la menace d’une situation problématique, des ressources, du potentiel, de la prédisposition, des besoins. Avec chaque enseignant, un contrat sera signé sur le modèle d’un contrat social. (KD3). Les actions envisagées dans le projet visent à accroître l’employabilité de l’apprentissage, ainsi qu’à prévenir l’exclusion sociale et la pauvreté. Grâce aux activités du projet, le taux minimum de performance sociale de 34 % sera atteint et le taux d’emploi d’au moins 22 % (KD5). Der Interventionsbereich ist die sozial-berufliche Aktivierung von 50 Personen. (35ki15m) in Gemeinden unterhalb der im Mazovian Barometer of Poverty and Social Exclusion (KMS2) aufgeführten Defowarisierung, die Sozialhilfeleistungen gemäß dem Gesetz vom 12. März 2004 über die Sozialhilfe oder Sozialhilfe erhalten, d. h. mindestens eine der Voraussetzungen des Artikels 7 dieses Gesetzes erfüllen. Die Unterstützung dieser Menschen wird auf dem für jede Person individuell geschaffenen Wiedereingliederungspfad beruhen, wobei die Diagnose einer Problemsituation oder die Bedrohung einer Problemsituation, Ressourcen, Potenziale, Prädisposition und Bedürfnisse berücksichtigt werden. Mit jedem Lehrer wird ein Vertrag im Sinne eines Sozialvertrags unterzeichnet. (KD3). Die im Projekt vorgesehenen Maßnahmen zielen darauf ab, die Beschäftigungsfähigkeit des Lernens zu verbessern und soziale Ausgrenzung und Armut zu verhindern. Als Ergebnis der Projektaktivitäten wird die Mindestsozialleistungsquote von 34 % erreicht, und die Beschäftigungsquote beträgt mindestens 22 % (KD5). Das Projekt wird vom 01.04.2018-31.12.2019 (21Monate)-(KD1) durchgeführt. Das Projekt wird unter PARTNERSTWIE:PRZEDSIĘBIORSTWO durchgeführt: Private – Akademie der Wissenschaften in Płońsk (AKN), Bildungs- und Beratungszentrum – INSTITATION OF THE WORK Market-Implementation – (LID) (KMS3). Mit dem Projekt werden Maßnahmen für Frauen und Männer und Nichtdiskriminierung durch gleichberechtigten Zugang zur Teilnahme am Projekt unabhängig von Geschlecht, Religion, Gesundheitszustand oder anderen Diskriminierungsgründen verfolgt. Das Projekt übernimmt folgende Aufgaben: 1. Schaffung eines individuellen Wiedereingliederungspfads,2. Unterzeichnung von Verträgen über das Modell eines sozialen Vertrags mit jedem Teilnehmer,3.Berufsberatung,4.Kurse, die den Erwerb von Computerqualifikationen ermöglichen,5.Kurse, die den Erwerb neuer Berufsqualifikationen ermöglichen,6.Family Academy,7.Professionelle Praktika.
