Настоящето проектно предложение е в пълно съответствие с Плана за действие за периода 2018-2021 г. за изпълнение на Националната стратегия за дългосрочна грижа. Планира се проекта да обхване най-малко 50 възрастни лица и хора с трайни увреждания, като за целта е създадено партньорство между общините Рила и Кочериново. Проектът включва предоставяне на почасови мобилни интегрирани здравно-социални услуги, провеждане на обучения и супервизия за персонала, както и закупуване на транспортно средство за осигуряване на мобилност при осъществяване на дейността. В резултат на реализация на дейността се очаква осигуряване на непрекъснатост на грижата, чрез участие на различни специалисти и заинтересовани лица, утвърждаване практиката на предоставяне на комплексна грижа чрез междусекторните услуги, които позволят по-добра индивидуализация на предлаганата подкрепа, по-точна оценка на потребностите и прилагане на мултидисциплинарен подход. This project proposal is fully in line with the 2018-2021 Action Plan for the implementation of the National Strategy for Long-Term Care. The project is planned to cover at least 50 adults and people with permanent disabilities, with a partnership between the municipalities of Rila and Kocherinovo. The project includes the provision of hourly mobile integrated health and social services, the provision of trainings and supervision for the staff, as well as the purchase of a means of transport to ensure mobility in the course of the activity. As a result of the implementation of the activity, it is expected to ensure continuity of care through the involvement of various professionals and stakeholders, the validation of the practice of providing complex care through the cross-sectoral services, which allow better individualisation of the support offered, a more accurate assessment of the needs and the application of a multidisciplinary approach. La présente proposition de projet est pleinement conforme au plan d’action 2018-2021 pour la mise en œuvre de la stratégie nationale pour les soins de longue durée. Le projet devrait couvrir au moins 50 adultes et personnes handicapées permanentes, avec un partenariat entre les municipalités de Rila et Kocherinovo. Le projet comprend la fourniture de services sociaux et de santé intégrés mobiles horaires, la fourniture de formations et de supervision pour le personnel, ainsi que l’achat d’un moyen de transport pour assurer la mobilité au cours de l’activité. Grâce à la mise en œuvre de l’activité, il est prévu d’assurer la continuité des soins par la participation de divers professionnels et parties prenantes, la validation de la pratique consistant à fournir des soins complexes à travers les services intersectoriels, ce qui permet une meilleure individualisation du soutien offert, une évaluation plus précise des besoins et l’application d’une approche multidisciplinaire.
