Celem głównym projektu jest ułatwienie powrotu do sprawności co najmniej 22% uczestników projektu - pacjentów (56os. 34K/22M), tak aby mogli oni podjąć pracę lub wydłużyć aktywność zawodową poprzez rehabilitację leczniczą w czasie trwania projektu (do 30.09.2020r.) i po jego zakończeniu. Główny rezultat projektu to podjęcie pracy lub kontynuowanie zatrudnienia przez 56 osoby. Grupa docelowa: Osoby w wieku aktywności zawodowej, powyżej 50 roku życia, ze zdiagnoznowaną chorobą układu kostno-mięśniowego i tkanki łącznej, w szczególności: - z przewlekłymi dolegliwościami bólowymi i/lub ograniczeniem ruchowości w przebiegu chorób zwyrodnieniowych stawów i innych artropatii, - z chorobami kręgosłupa, - z chorobami tkanek miękkich (mięśni, powięzi i ścięgien), których stan zdrowia umożliwia podjęcie rehabilitacji w trybie ambulatoryjnym. Zadania projektowe: - Zakup wyposażenia - Realizacja programu rehabilitacji leczniczej dla 250 pacjentów. (Polish)

The main objective of the project is to facilitate the return to efficiency of at least 22 % of project participants – patients (56 persons). 34K/22M) so that they can take up work or prolong their professional activity by means of therapeutic rehabilitation during the project (until 30.09.2020) and after the completion of the project. The main result of the project is to take up a job or continue to employ 56 people. Target group: Persons over 50 years of age with a diagnosis of musculoskeletal and connective tissue disease, in particular: — with chronic pain and/or reduced mobility in the course of degenerative diseases of the joints and other arthropathy, – with diseases of the spine, – with soft tissue diseases (muscles, fascia and tendons), the condition of which allows to undergo an outpatient rehabilitation. Project tasks: — Purchase of equipment – Implementation of a therapeutic rehabilitation programme for 250 patients. (English)

L’objectif principal du projet est de faciliter la récupération d’au moins 22 % des participants au projet — les patients (56 personnes). 34K/22M) afin qu’ils puissent prendre un emploi ou prolonger l’activité professionnelle par le biais de la réhabilitation thérapeutique pendant le projet (jusqu’au 30.9.2020) et après son achèvement. Le principal résultat de ce projet est la reprise du travail ou la poursuite de l’emploi pour 56 personnes. Groupe cible: Les personnes âgées de plus de 50 ans de travail atteintes d’une maladie du tissu osseux-musculaire et du tissu conjonctif diagnostiquée, en particulier: — avec douleur chronique et/ou mobilité réduite au cours de l’arthrose et d’autres arthrites, — avec des maladies de la colonne vertébrale, — avec des maladies des tissus mous (muscle, fascia et tendons), dont l’état de santé permet d’entreprendre une réadaptation ambulatoire. Hauptziel des Projekts ist es, die Genesung von mindestens 22 % der Projektteilnehmer – Patienten (56 Personen) – zu erleichtern. 34K/22M), damit sie während des Projekts (bis zum 30.09.2020) und nach Abschluss des Projekts (bis 30.09.2020) durch therapeutische Rehabilitation eine Arbeit aufnehmen oder ihre berufliche Tätigkeit verlängern können. Das Hauptergebnis des Projekts ist die Aufnahme einer Arbeit oder die Fortsetzung einer Beschäftigung für 56 Personen. Zielgruppe: Personen im Alter von über 50 Jahren mit diagnostizierter Knochenmuskulatur- und Bindegewebeerkrankung, insbesondere: — mit chronischen Schmerzen und/oder eingeschränkter Mobilität im Verlauf von Osteoarthritis und anderen Arthritis, – mit Erkrankungen der Wirbelsäule, – mit Weichgewebeerkrankungen (Muskel, Faszien und Sehnen), deren Gesundheitszustand eine ambulante Rehabilitation ermöglicht. Projektaufgaben: — Kauf von Ausrüstung – Durchführung eines kurativen Rehabilitationsprogramms für 250 Patienten. (German)
