Cel projektu: Poprawa sytuacji życiowej 30 osób niesamodzielnych z terenu Gminy Dobrcz poprzez zwiększenie dostępności do wysokiej jakości usług opiekuńczych świadczonych w społeczności lokalnej i wzrost umiejętności 30 opiekunów faktycznych w zakresie opieki nad osobami niesamodzielnymi do 31.03.2020 r. Projekt zakłada formalne partnerstwo JST (Gmina Dobrcz) i uczelni / organizacji non-profit Wyższa Szkoła Gospodarki w Bydgoszczy. Projekt adresowany jest do osób zamieszkujący na terenie gminy Dobrcz, w tym 30 osób niesamodzielnych zagrożonych ubóstwem lub wykluczeniem społecznym w rozumieniu regulaminu konkursu i 30 opiekunów faktycznych i osób z otoczenia osób niesamodzielnych. Główne zadania to: Stworzenie 15 miejsc usług opiekuńczych świadczonych w społ. lokalnej na piętrze piętrze Domu Pomocy Społecznej w Gądeczu. W tym celu zakłada się: prace adaptacyjne, zakup wyposażenia i pełne dostosowanie przestrzeni do potrze DDP. Prowadzenie dziennego domu pobytu dla osób niesamodzielnych świadczącego standardowe i specjalistyczne usługi opiekuńcze dobrane w sposób kompleksowy i zindywidualizowany do potrzeb uczestników (w zakresie nieobjętym ustawą z 27.08.2004 r. o świad. opieki zdrowotnej). Zapewnienie wsparcia opiekunom faktycznym i osobom z otoczenia osób niesamodzielnych . Adaptacja umożliwi świadczenie pełnych dedykowanych form opieki i wsparcia dla osób z różnymi potrzebami i ograniczeniami samodzielności oraz będzie zgodna z wymogami stawianymi Regulaminem Konkursu. Objective of the project: Improving the living situation of 30 independent people from the area of Dobrcz commune by increasing access to high quality care services provided in the local community and increasing the skills of 30 actual carers in the field of care for dependent persons by 31.03.2020. The project assumes a formal partnership of JST (Commune of Dobrcz) and universities/non-profit organisations of the University of Economy in Bydgoszcz. The project is addressed to people living in the municipality of Dobrcz, including 30 dependent persons at risk of poverty or social exclusion within the meaning of the rules of the competition and 30 actual carers and persons from the environment of dependent persons. The main tasks are: Creation of 15 places of care services provided in the local community on the floor of the House of Social Welfare in Gądecz. To this end, it shall be assumed to: adaptation work, purchase of equipment and full adaptation of the space to the DDP. Running a daily home for dependants providing standard and specialised care services chosen in a comprehensive and personalised manner to the needs of the participants (within the scope not covered by the Act of 27.08.2004 on the Health Care Witness). Provide support to actual caregivers and non-reliant people. The adaptation will make it possible to provide full dedicated forms of care and support for people with different needs and limitations of independence and will comply with the requirements of the Competition Rules. Objectif du projet: Améliorer la situation de 30 personnes à charge de la municipalité de Dobrcz en augmentant la disponibilité de services de soins de qualité au sein de la communauté locale et en renforçant les compétences de 30 personnes qui s’occupent de personnes dépendantes d’ici au 31 mars 2020. Le projet établit un partenariat formel entre la JST (Commune Dobrcz) et une université/organisation à but non lucratif de l’École supérieure d’économie de Bydgoszcz. Le projet s’adresse aux personnes vivant dans la municipalité de Dobrcz, dont 30 personnes à charge exposées au risque de pauvreté ou d’exclusion sociale au sens des règles du concours, ainsi qu’à 30 personnes qui s’occupent effectivement de personnes à charge et qui viennent des environs de personnes dépendantes. Les principales tâches sont les suivantes: Création de 15 places de services de prise en charge dans une zone locale à l’étage de la maison de protection sociale de Gądek. À cette fin, les éléments suivants sont pris en considération: travaux d’adaptation, achat d’équipements et adaptation complète de l’espace au DDP. Gestion quotidienne d’un foyer pour personnes à charge offrant des services de soins standard et spécialisés sélectionnés de manière complète et individualisée en fonction des besoins des participants (dans la mesure où la loi du 27 août 2004 sur les témoins en matière de soins de santé n’est pas couverte). Fournir un soutien aux aidants naturels et aux personnes à charge. L’adaptation permettra la fourniture de formes de soins et de soutien entièrement spécifiques aux personnes ayant des besoins et des limitations d’indépendance différents et se conformera aux exigences des règles de concurrence.
