Predkladaným projektom Mesto Stará Ľubovňa reaguje na potrebu poskytovania opatrovateľskej služby vo forme, ktorá najmenej zasahuje a mení zabehnutý spôsob života ľudí, ktorí sa v dôsledku nepriaznivej sociálnej situácie z dôvodu ťažkého zdravotného postihnutia, nepriaznivého zdravotného stavu alebo z dôvodu dovŕšenia dôchodkového veku stali odkázanými na pomoc inej osoby. Projekt je zameraný na cieľovú skupinu ktorou sú deti (maloleté osoby) a plnoleté fyzické osoby, ktorým sa poskytujú sociálne služby a zamestnanci vykonávajúci politiky a opatrenia v oblasti sociálneho začlenenia vo verejnom aj neverejnom sektore. Prioritou projektu je zlepšenie prístupu k cenovo prístupným, trvalo udržateľným a kvalitným službám vrátane zdravotnej starostlivosti a sociálnych služieb všeobecného záujmu a tým podporiť postupný prechod z inštitucionálnej starostlivosti mimo prirodzeného prostredia osoby, ktorej sa starostlivosť poskytuje na komunitnú starostlivosť poskytovanú v prostredí domova, kedy nedochádza k segregácii a izolácii človeka od jeho „bežného sveta“, čím by sa napĺňal cieľ projektu a to prechod z inštitucionálnej na komunitnú starostlivosť. Pri inštitucionálnej starostlivosti v zariadení sociálnych služieb je človek nútený prispôsobiť sa prevádzke a určitému, vopred navrhnutému, programu v danom zariadení, čím sa táto osoba svojim spôsobom vzdáva možnosti rozhodovania o tom, ako prežije svoj deň. Premiestnením do zariadenia sa ľudia izolujú od svojich blízkych, od života, ktorý dobre poznajú a častokrát strácajú nádej, že sa raz navrátia k svojmu doterajšiemu spôsobu žitia, čím dochádza k podstatnému zníženiu kvality ich života. Hlavnou aktivitou projektu preto je poskytovanie opatrovateľskej služby v zmysle ustanovení zákona č. 448/2008 Z.z. o sociálnych službách , ako domácej opatrovateľskej služby poskytovanej osobe v jej prirodzenom domácom prostredí. Stanovený cieľ projektu, prechod z inštitucionálnej na komunitnú starostlivosť, plánuje Mesto naplniť realizáciou tohto predkladaného projektu, v rámci ktorého bude opatrovateľskú službu v zmysle ustanovení zákona o sociálnych službách č. 448/2008 v zmysle § 12 ods. 1 písm. c) bod 2, § 13 ods. 3 a § 41 vykonávať 15 kvalifikovaných opatrovateľov/opatrovateliek na ustanovený týždenný pracovný čas, čím sa naplní počet merateľného ukazovateľa projektu „Počet podporovaných kapacít nových, inovatívnych služieb alebo opatrení na komunitnej úrovni, v domácom prostredí, otvorenom prostredí alebo náhradnom prostredí“ stanovený na cieľovú hodnotu 15 a merateľný ukazovateľ „Počet projektov zameraných na verejné správy alebo sociálne služby na vnútroštátnej, regionálnej a miestnej úrovni“, ktorého cieľovou hodnotou v projekte je hodnota 1. Projekt bude realizovaný v domácom prostredí klientov na celom území Mesta Stará Ľubovňa.Realizáciou projektu by nedochádzalo k segregácií ľudí, ktorí sú odkázaní na pomoc inej osoby, tým že by sa im zlepšil prístup k cenovo prístupným, trvalo udržateľným a kvalitným službám vrátane zdravotnej starostlivosti a sociálnych služieb všeobecného záujmu. Tieto opatrované osoby by ostali sociálne začlenené vo svojom doterajšom sociálnom prostredí. Šírením informácie o raelizácii tejto činnosti a možnosti jej využívania bude podporený postupný prechod z inštitucionálnej na komunitnú starostlivosť na území Mesta Stará Ľubovňa. (Slovak)

With this project, the City of Stará Ľubovňa responds to the need to provide care services in a form that least interferes with and alters the established way of life of people who have become dependent on the help of another person as a result of an unfavourable social situation due to severe disability, unfavorable health status or retirement age. The project focuses on the target group of children (minors) and adults who are provided with social services and employees implementing policies and measures in the field of social inclusion in both the public and non-public sectors. The priority of the project is to improve access to affordable, sustainable and quality services, including health care and social services of general interest, thereby supporting the gradual transition from institutional care outside the natural environment of the person receiving care to community-based care provided in a home environment where there is no segregation and isolation from his or her “normal world”, thereby fulfilling the project’s objective of moving from institutional to community-based care. In the case of institutional care in a social service facility, a person is forced to adapt to the operation and to a certain pre-designed programme in a given establishment, thus giving up, in his own way, the possibility of deciding how to survive his day. By moving to the facility, people are isolated from their loved ones, from a well-known life that they often lose hope of returning to their existing living patterns, thus significantly reducing their quality of life. The main activity of the project is therefore the provision of care services in accordance with the provisions of Act No 448/2008 on social services, as a home care service provided to a person in their natural home environment. The aim of the project, namely the transition from institutional to community-based care, is to be met by the City by carrying out this proposed project, in which 15 qualified carers/carers will be carried out for the prescribed weekly working time, within the meaning of Section 12(1)(c)(2), Section 13(3) and Section 41, in accordance with the provisions of the Social Services Act No 448/2008, thus fulfilling the number of measurable indicators of the project "Number of supported capacities of new, innovative services or measures at community, home, open or substitute level, set at target 15 and a measurable indicator ‘Number of projects targeting public administrations or social services at national, regional and local level’, the target value of which in the project is value 1. The project will be implemented in the home environment of clients throughout the city of Stará Ľubovňa.The implementation of the project would not result in segregation of people who are dependent on the help of another person, by improving their access to affordable, sustainable and quality services, including health care and social services of general interest. These carers would remain socially integrated in their existing social environment. By disseminating information about the raelisation of this activity and the possibility of its use, the gradual transition from institutional to community-based care in the town of Stará Ľubovňa will be supported. Avec ce projet, la ville de Stará ▲ubovňa répond à la nécessité de fournir des services de soins sous une forme qui interfère le moins avec et modifie le mode de vie établi des personnes qui sont devenues dépendantes de l’aide d’une autre personne en raison d’une situation sociale défavorable en raison d’un handicap grave, de l’état de santé défavorable ou de l’âge de la retraite. Le projet est axé sur le groupe cible d’enfants (mineurs) et d’adultes qui reçoivent des services sociaux et des employés qui mettent en œuvre des politiques et des mesures dans le domaine de l’inclusion sociale, tant dans le secteur public que dans le secteur non public. La priorité du projet est d’améliorer l’accès à des services abordables, durables et de qualité, y compris des soins de santé et des services sociaux d’intérêt général, facilitant ainsi la transition progressive des soins en institution en dehors de l’environnement naturel de la personne qui reçoit des soins à des soins de proximité dispensés dans un environnement à domicile où il n’y a pas de ségrégation et d’isolement de son «monde normal», ce qui permettra d’atteindre l’objectif du projet de passer des soins en établissement aux soins de proximité. Dans le cas d’une prise en charge institutionnelle dans un établissement de services sociaux, une personne est contrainte de s’adapter à l’opération et à un certain programme préconçu dans un établissement donné, abandonnant ainsi, à sa manière, la possibilité de décider comment survivre à sa journée. En déménageant à l’établissement, les gens sont isolés de leurs proches, d’une vie bien connue qu’ils perdent souvent l’espoir de revenir à leurs modes de vie existants, ce qui réduit considérablement leur qualité de vie. L’activité principale du projet est donc la fourniture de services de soins conformément aux dispositions de la loi no 448/2008 sur les services sociaux, en tant que service de soins à domicile fourni à une personne dans son environnement naturel à domicile. L’objectif du projet, à savoir le passage de la prise en charge en institution à la prise en charge communautaire, doit être atteint par la Ville en réalisant le projet proposé, dans lequel 15 aidants/soignants qualifiés seront effectués pour la durée hebdomadaire de travail prescrite, au sens de l’article 12, paragraphe 1, point c) 2), de l’article 13, paragraphe 3, et de l’article 41, conformément aux dispositions de la loi sur les services sociaux no 448/2008, satisfaisant ainsi au nombre d’indicateurs mesurables du projet "Nombre de capacités soutenues de nouveaux, services ou mesures innovants au niveau communautaire, national, ouvert ou de remplacement, fixés à l’objectif 15 et un indicateur mesurable "Nombre de projets ciblant les administrations publiques ou les services sociaux au niveau national, régional et local", dont la valeur cible est la valeur 1. Le projet sera mis en œuvre dans l’environnement domestique des clients dans toute la ville de Stará ▲ubovňa. La mise en œuvre du projet n’entraînerait pas de ségrégation des personnes dépendantes de l’aide d’une autre personne, en améliorant leur accès à des services abordables, durables et de qualité, y compris des soins de santé et des services sociaux d’intérêt général. Ces aidants resteraient socialement intégrés dans leur environnement social existant. En diffusant des informations sur la raélisation de cette activité et sur la possibilité de son utilisation, le passage progressif de la prise en charge institutionnelle à la prise en charge communautaire dans la ville de Stará Uubovňa sera soutenu. Mit diesem Projekt reagiert die Stadt Stará Œubovňa auf die Notwendigkeit, Betreuungsleistungen in einer Form zu erbringen, die die etablierte Lebensweise von Menschen, die aufgrund einer ungünstigen sozialen Situation aufgrund schwerer Behinderung, ungünstiger Gesundheitszustand oder Ruhestandsalter von der Hilfe einer anderen Person abhängig geworden sind, am wenigsten beeinträchtigt und verändert. Das Projekt konzentriert sich auf die Zielgruppe von Kindern (Minderjähriger) und Erwachsenen, die soziale Dienstleistungen und Mitarbeiter erhalten, die Strategien und Maßnahmen im Bereich der sozialen Eingliederung sowohl im öffentlichen als auch im nichtöffentlichen Bereich umsetzen. Priorität des Projekts ist es, den Zugang zu erschwinglichen, nachhaltigen und hochwertigen Dienstleistungen, einschließlich Gesundheitsversorgung und Sozialdienstleistungen von allgemeinem Interesse, zu verbessern und so den schrittweisen Übergang von der institutionellen Betreuung außerhalb des natürlichen Umfelds der Person, die in einem häuslichen Umfeld erbracht wird, in dem es keine Trennung und Isolierung von ihrer „normalen Welt“ gibt, zu fördern und so das Ziel des Projekts zu erfüllen, von institutioneller zu gemeinschaftsbasierter Betreuung zu wechseln. Im Falle der institutionellen Betreuung in einer Sozialdiensteinrichtung ist eine Person gezwungen, sich an die Operation und ein bestimmtes vorgefertigtes Programm in einer bestimmten Einrichtung anzupassen und damit auf seine eigene Art und Weise die Möglichkeit aufzugeben, zu entscheiden, wie man seinen Tag überlebt. Durch den Umzug in die Anlage werden Menschen von ihren Angehörigen isoliert, von einem bekannten Leben, das sie oft die Hoffnung verlieren, zu ihren bestehenden Lebensmustern zurückzukehren, wodurch ihre Lebensqualität deutlich reduziert wird. Die Haupttätigkeit des Projekts ist daher die Erbringung von Betreuungsleistungen gemäß den Bestimmungen des Gesetzes Nr. 448/2008 über soziale Dienstleistungen als häusliche Pflegeleistung, die einer Person in ihrem natürlichen häuslichen Umfeld erbracht wird. Das Ziel des Projekts, nämlich der Übergang von der institutionellen zur gemeindenahen Betreuung, wird von der Stadt durch die Durchführung dieses vorgeschlagenen Projekts erreicht, bei dem 15 qualifizierte Pflegekräfte/Betreuer für die vorgeschriebene wöchentliche Arbeitszeit im Sinne von § 12 Abs. 1 Buchst. c Abs. 2, § 13 Abs. 3 und § 41 gemäß den Bestimmungen des Sozialdienstleistungsgesetzes Nr. 448/2008 durchgeführt werden und damit die Anzahl messbarer Indikatoren des Projekts "Anzahl der unterstützten Kapazitäten neuer, innovative Dienstleistungen oder Maßnahmen auf kommunaler, häuslicher, offener oder substituierter Ebene, die auf Ziel 15 und einen messbaren Indikator "Anzahl der Projekte für öffentliche Verwaltungen oder soziale Dienste auf nationaler, regionaler und lokaler Ebene" festgelegt sind, deren Zielwert im Projekt Wert 1 ist. Das Projekt wird im häuslichen Umfeld von Kunden in der ganzen Stadt Stará Œubovňa durchgeführt.Die Durchführung des Projekts würde nicht zu einer Segregation von Menschen führen, die auf die Hilfe einer anderen Person angewiesen sind, indem ihr Zugang zu erschwinglichen, nachhaltigen und hochwertigen Dienstleistungen, einschließlich Gesundheitsversorgung und Sozialdienstleistungen von allgemeinem Interesse, verbessert wird. Diese Betreuer würden weiterhin sozial in ihr bestehendes soziales Umfeld integriert sein. Durch die Verbreitung von Informationen über die Raelisierung dieser Tätigkeit und die Möglichkeit ihrer Nutzung wird der schrittweise Übergang von der institutionellen Betreuung in der Stadt Stará Œubovňa unterstützt.
