Namen projekta je izboljšanje pogojev za primarno zdravstveno oskrbo na območju Upravne enote Trebnje in sicer z investiranjem v ustrezne kapacitete, ki bodo omogočale krepitev javnozdravstvene vloge zdravstvenega doma Trebnje, implementacijo nadgradnje preventivnih pregledov za otroke in mladostnike, izvajanje preventivnih obravnav za ogrožene otroke in mladostnike, izvajanje prenovljene in nadgrajene preventive kroničnih bolezni, vključevanje ranljivih oseb v preventivno zdravstveno varstvo ter uvajanje modela skupnostnega pristopa za krepitev zdravja in zmanjševanje neenakosti v zdravju na območju Upravne enote Trebnje. Vse navedene aktivnosti se bodo izvajale preko vzpostavitve integriranih centrov za krepitev zdravja (CKZ) kot samostojnih enot v zdravstvenem domu Cilj projekta je zagotovitev prostorskih in materialnih pogojev za izvajanje dejavnosti centra za krepitev zdravja. The purpose of the project is to improve the conditions for primary health care in the area of Trebnje Administrative Unit by investing in appropriate capacities that will enable the strengthening of the public health role of the Trebnje health centre, the implementation of the upgrading of preventive examinations for children and adolescents, the implementation of preventive treatments for children and adolescents at risk, the implementation of renewed and upgraded prevention of chronic diseases, the inclusion of vulnerable persons in preventive healthcare and the introduction of a Community approach model for health promotion and reduction of health inequalities in the area of the Trebnje Administrative Unit. All the above activities will be carried out through the establishment of integrated health promotion centres (CKZ) as separate units in the health centre The objective of the project is to provide spatial and material conditions for the implementation of the activities of the Centre for Health Promotion.
