    EL SINDROME DE DISFUNCION MULTIORGANICA (SDMO) ES UN TRASTORNO FISIOLOGICO (POTENCIALMENTE REVERSIBLE) ENTRE DOS O MAS SISTEMAS DE ORGANOS QUE SURGE A RAIZ DE UN INSULTO FISIOLOGICO POTENCIALMENTE PELIGROSO PARA LA VIDA. HISTORICAMENTE, EL TRATAMIENTO Y CUIDADO DEL PACIENTE SDMO POR PARTE DE LOS FACULTATIVOS EN LAS UNIDAD DE CUIDADOS INTENSIVOS (UCI) SE HA CENTRADO EN SU SUPERVIVENCIA, PERO EN LA ULTIMA DECADA EXISTE SUFICIENTE EVIDENCIA CLINICA DE QUE LOS PACIENTES QUE SOBREVIVEN AL SDMO MANIFIESTAN UN DETERIORO COGNITIVO Y NEUROLOGICO QUE PERMANECE ESTABLE INCLUSO HASTA VARIOS AÑOS DESPUES DE RECIBIR EL ALTA EN LA UCI. HASTA EL DIA DE HOY SON POCOS LOS ESTUDIOS LONGITUDINALES QUE SE HAN REALIZADO CON EL FIN DE DETERMINAR LAS CARACTERISTICAS COGNITIVAS Y LOS CAMBIOS A NIVEL DE REDES CEREBRALES DEPUES DEL ALTA DE ESTOS PACIENTES EN LA UCI. POR TAL MOTIVO, EN ESTE PROYECTO PRESENTAMOS UN ESTUDIO OBSERVACIONAL, PROSPECTIVO Y LONGITUDINAL CON DURACION DE 36 MESES DONDE SE ESTUDIARA LA EVOLUCION DEL DETERIORO COGNITIVO DEL PACIENTE SDMO. EN PARTICULAR, SE PRETENDE EVALUAR ESTOS PACIENTES A LOS 6 Y 12 MESES DESPUES DE RECIBIR EL ALTA EN LA UCI MEDIANTE DIFERENTE TESTS NEUROPSICOLOGICOS (QUE MIDEN RESPUESTA EN MEMORIA, ORGANIZACION PERCEPTUAL, ATENCION, FUNCION EJECUTIVA, RAZONAMIENTO ABSTRACTO O FLUIDEZ VERBAL) Y MEDIANTE RESONANCIA MAGNETICA PERMITIENDO ANALIZAR LA PROGRESION REDIOLOGICA (COMO ATROFIA CORTICAL Y SUBCORTICAL, INCREMENTO DE VOLUMEN VENTRICULAR, ASI COMO VARIACIONES EN INTEGRIDAD DE LA MATERIA BLANCA) Y LAS VARIACIONES EN LAS REDES DEL CEREBRO, TANTO ESTRUCTURALES OBTENIDAS A PARTIR DE IMAGENES DEL TENSOR DE DIFUSION COMO FUNCIONALES, OBTENIDAS A PARTIR DE SECUENCIAS T2* EN EL ESTADO DE REPOSO. SE PRETENDE RECLUTAR DURANTE LA TOTALIDAD DEL PROYECTO A N=60 PACIENTES SDMO Y N=60 CONTROLES DE POBLACION SANA (BALANCEADO POR SEXO Y EDAD AL GRUPO DE PACIENTES). LOS OBJETIVOS DEL PRESENTE ESTUDIO SON: 1) DETERMINAR SI EXISTEN DIFERENCIAS SIGNIFICATIVAS EN EL RENDIMIENTO COGNITIVO ENTRE EL GRUPO DE CONTROL Y EL GRUPO DE PACIENTES SDMO A LOS 6 Y 12 MESES DESPUES DEL ALTA EN LA UCI. 2) DETERMINAR QUE FUNCIONES COGNITIVAS Y QUE MARCADORES NEUROIMAGEN SON ESTABLES EN EL TIEMPO, CUALES EMPEORAN Y CUALES MEJORAN DESDE LOS 6 MESES DESPUES AL ALTA EN LA UCI HASTA LOS 12 MESES DESPUES EN LOS PACIENTES SDMO, Y COMPARAR SU EVOLUCION CON LA DEL GRUPO CONTROL. 3) DETERMINAR LA RELACION EXISTENTE ENTRE LAS VARIABLES CLINICAS MEDIDAS DURANTE LA ESTANCIA EN LA UCI (COMO LA DURACION EN DELIRIO, LA CANTIDAD DE SEDACION Y ANALGESICOS ADMINISTRADOS O DIFERENTES PARAMETROS BIOQUIMICOS) Y SU RENDIMIENTO COGNITIVO A LOS 6 Y 12 MESES DESPUES DE SU ALTA. ESPERAMOS QUE LOS RESULTADOS DE ESTE ESTUDIO PUEDAN SERVIR EN UN FUTURO PARA DETERMINAR CUALES SON LAS VARIABLES CLINICAS IMPLICADAS EN LA APARICION DE CAMBIOS COGNITIVOS Y EN LA CONECTIVIDAD CEREBRAL DE LOS PACIENTES SDMO, ASI COMO PARA DETERMINAR LA INCIDENCIA DEL DETERIORO COGNITVO EN ESTOS PACIENTES, CON LA FINALIDAD DE PODER AYUDAR A ESTABLECER MEJORES PROTOCOLOS DE TRATAMIENTO Y MANEJO CLINICO DE ESTOS PACIENTES AL MOMENTO DEL INGRESO A LA UCI . HISTORICALLY, THE TREATMENT AND CARE OF THE SDMO PATIENT BY PHYSICIANS IN THE INTENSIVE CARE UNIT (ICU) HAS FOCUSED ON THEIR SURVIVAL, BUT IN THE LAST DECADE THERE IS SUFFICIENT CLINICAL EVIDENCE THAT PATIENTS WHO SURVIVE SDMO MANIFEST COGNITIVE AND NEUROLOGICAL IMPAIRMENT THAT REMAINS STABLE EVEN FOR SEVERAL YEARS AFTER DISCHARGE FROM THE ICU. TO DATE, FEW LONGITUDINAL STUDIES HAVE BEEN CARRIED OUT TO DETERMINE COGNITIVE CHARACTERISTICS AND CHANGES IN BRAIN NETWORKS AFTER THE DISCHARGE OF THESE PATIENTS INTO THE ICU. FOR THIS REASON, IN THIS PROJECT WE PRESENTED AN OBSERVATIONAL, PROSPECTIVE AND LONGITUDINAL STUDY LASTING 36 MONTHS TO STUDY THE EVOLUTION OF COGNITIVE IMPAIRMENT OF THE PATIENT SDMO. IN PARTICULAR, THE AIM IS TO EVALUATE THESE PATIENTS AT 6 AND 12 MONTHS AFTER BEING DISCHARGED FROM THE ICU BY MEANS OF DIFFERENT NEUROPSYCHOLOGIC TESTS (MEASURING RESPONSE IN MEMORY, PERCEPTUAL ORGANISATION, ATTENTION, EXECUTIVE FUNCTION, ABSTRACT REASONING OR VERBAL FLUIDITY) AND BY MAGNETIC RESONANCE ALLOWING ANALYSIS OF REDIOLOGICAL PROGRESSION (SUCH AS CORTICAL AND SUBCORTICAL ATROPHY, INCREASE IN VENTRICULAR VOLUME, AS WELL AS VARIATIONS IN THE INTEGRITY OF WHITE MATTER) AND VARIATIONS IN THE NETWORKS OF THE BRAIN, BOTH STRUCTURALLY OBTAINED FROM IMAGES OF THE DIFUSION TENSIONER AND FUNCTIONAL, OBTAINED FROM T2* SEQUENCES IN THE STATE OF REST. THE AIM IS TO RECRUIT N=60 SDMO PATIENTS AND N=60 HEALTHY POPULATION CONTROLS THROUGHOUT THE PROJECT (BALANCED BY SEX AND AGE TO THE PATIENT GROUP). THE OBJECTIVES OF THIS STUDY ARE TO: 1) DETERMINE WHETHER THERE ARE SIGNIFICANT DIFFERENCES IN COGNITIVE PERFORMANCE BETWEEN THE CONTROL GROUP AND THE GROUP OF SDMO PATIENTS AT 6 AND 12 MONTHS AFTER DISCHARGE FROM THE ICU. 2) DETERMINE THAT COGNITIVE FUNCTIONS AND NEUROIMAGE MARKERS ARE STABLE OVER TIME, WHICH WORSEN AND WHICH IMPROVE FROM 6 MONTHS AFTER ICU DISCHARGE TO 12 MONTHS LATER IN SDMO PATIENTS, AND COMPARE THEIR EVOLUTION WITH THAT OF THE CONTROL GROUP. 3) DETERMINE THE RELATIONSHIP BETWEEN THE CLINICAL VARIABLES MEASURED DURING THE STAY IN THE ICU (SUCH AS THE DURATION IN DELIRIUM, THE AMOUNT OF SEDATION AND ANALGESICS ADMINISTERED OR DIFFERENT BIOCHEMICAL PARAMETERS) AND THEIR COGNITIVE PERFORMANCE AT 6 AND 12 MONTHS AFTER DISCHARGE. WE HOPE THAT THE RESULTS OF THIS STUDY CAN SERVE IN THE FUTURE TO DETERMINE THE CLINICAL VARIABLES INVOLVED IN THE APPEARANCE OF COGNITIVE CHANGES AND BRAIN CONNECTIVITY OF SDMO PATIENTS, AS WELL AS TO DETERMINE THE INCIDENCE OF COGNITIVE IMPAIRMENT IN THESE PATIENTS, IN ORDER TO HELP ESTABLISH BETTER TREATMENT PROTOCOLS AND CLINICAL MANAGEMENT OF THESE PATIENTS UPON ADMISSION TO THE ICU.
