Cilj projekta je krepitev javnozdravstvene vloge zdravstvenih domov (v nadaljevanju ZD), implementacija nadgradenj preventivnih pregledov za otroke in mladostnike, izvajanje preventivnih obravnav za ogrožene otroke in mladostnike ter izvajanje integrirane preventive kroničnih bolezni (v nadaljevanju KB) na primarni ravni zdravstvenega varstva, vključevanje ranljivih oseb v preventivno zdravstveno varstvo ter uvajanje modela skupnostnega pristopa za krepitev zdravja in zmanjševanje neenakosti v zdravju v lokalnih skupnostih. Prav tako je namen projekta preko zagotavljanja dodatnih strokovnih izobraževanj in usposabljanj s področja javnega zdravja, zmanjševanja neenakosti v zdravju in kulturnih kompetenc opolnomočiti izvajalce preventivnih obravnav za vse populacijske skupine iz izbranih 25 ZD in pripadajočih lokalnih skupnostih za izvajanje nadgradenj preventivnih programov, s posebnim poudarkom na pridobivanju kulturnih kompetenc in vključevanju ranljivih oseb v preventivne programe. The aim of the project is to strengthen the public health role of health centres (hereinafter ZD), to implement upgrades of preventive examinations for children and adolescents, to carry out preventive treatments for children and adolescents at risk and to implement integrated prevention of chronic diseases (hereinafter: KB) at the primary level of health care, to include vulnerable persons in preventive healthcare and to introduce a Community approach model for health promotion and reduction of health inequalities in local communities. By providing additional professional education and training in the field of public health, reducing health inequalities and cultural competences, the project aims to empower practitioners of preventive treatments for all population groups from selected 25 ZDs and their local communities to carry out upgrading prevention programmes, with special emphasis on the acquisition of cultural competences and the inclusion of vulnerable people in prevention programmes.
