    Se pretende, mediante el uso de apps y warables, fomentar la activación de los pacientes para mejorar sus prácticas de autocuidado en personas con enfermedad crónica (cardiovasculares), mayores de 40 años con 2 o más fármacos diarios, donde se concentran los problemas de incumplimiento, errores de medicación del paciente y mayores dificultades para el control de factores de riesgo. Se diseña un estudio experimental (intervención aleatorizado, con 18 meses de seguimiento) con 2 grupos y medidas repetidas antes-después. Para el consenso de los planes estandarizados de autocuidado, según perfiles de pacientes, se aplicará la técnica de conferencia de consenso. La efectividad de la App se mide en términos de: grado de control de los factores de riesgo, autoeficacia en autocuidado, frecuencia de conductas saludables (ejercicio físico, dieta), cumplimiento terapéutico, uso seguro de la medicación, frecuentación (programada, demanda y urgente) y coste de la asistencia.
