La enfermedad renal y cardiovascular (CV) comparten factores de riesgo comunes. La presencia de ambas enfermedades incrementa significativamente el riesgo de eventos CV y de muerte tras ajustarse por los factores de riesgo convencionales. Una de las poblaciones donde de manera más preocupante se da esta situación es en los pacientes en diálisis, donde el riesgo CV permanece muy elevado. En estos pacientes 2/3 de las muertes cardiacas son atribuibles a arritmias fatales, siendo la muerte súbita la responsable del 25% de la mortalidad total. Los niveles séricos de fosfatos elevados son un factor de riesgo no convencional en los pacientes con enfermedad renal. Sin embargo, asociado a ello existe un “actor secundario” denominado factor de crecimiento de fibroblastos (FGF)-23 que podría tener una repercusión CV incluso mayor que el ambiente hiperfosfatémico. En la enfermedad renal los niveles de FGF-23 se van incrementando a medida que la capacidad funcional del riñón para excretar los fosfatos va disminuyendo. Recientemente, niveles altos de FGF-23 se han relacionado con una mayor probabilidad de eventos CV y muerte en patologías cardiacas como la insuficiencia. Actualmente se desconoce si FGF-23 podría jugar un papel determinante en las alteraciones del ritmo cardiaco que pudiera explicar el alto porcentaje de eventos CV fatales que presentan los pacientes en diálisis. Realizaremos un abordaje traslacional aunando investigación clínica en pacientes en diálisis y experimental en el laboratorio con el objetivo de legitimar a FGF-23 como nuevo biomarcador de riesgo CV en la enfermedad crónica renal terminal. Kidney and cardiovascular disease (CV) share common risk factors. The presence of both diseases significantly increases the risk of CV events and death after adjustment for conventional risk factors. One of the populations where this situation is most worrying is in dialysis patients, where the CV risk remains very high. In these patients 2/3 of cardiac deaths are attributable to fatal arrhythmias, with sudden death responsible for 25 % of total mortality. Elevated serum phosphate levels are an unconventional risk factor in patients with kidney disease. However, associated with this is a “secondary agent” called fibroblast growth factor (FGF)-23 that could have an even higher CV impact than the hyperphosphatamic environment. In kidney disease FGF-23 levels are increasing as the functional ability of the kidney to excrete phosphates decreases. Recently, high levels of FGF-23 have been associated with a higher probability of CV events and death in cardiac pathologies such as insufficiency. It is currently unknown whether FGF-23 could play a determining role in heart rhythm alterations that could explain the high percentage of fatal CV events presented by dialysis patients. We will perform a translational approach combining clinical research in dialysis and experimental patients in the laboratory with the aim of legitimising FGF-23 as a new CV risk biomarker in end-term chronic renal disease.
