GRUPA DOCELOWA: 200 (120K,80M) os biernych zawodowo, zagrożonych ubóstwem lub wykluczeniem społecznym, zamieszk. zg z KC naobszarze woj. wielkopolskiego, będących:- os. korzystającymi ze świadczeń z pomocy społecznej zg. z ustawą z 12.03.2004 r. o pomocy społ. lub kwalifikujące się do objęcia wsparciem pomocyspoł.- z tytułu ubóstwa (tj. spełniające przesłankę ubóstwa zg. z art.7 ww.ustawy),i/lub- os. z niepełnoprawnościami, w tym min.10% uczestników to os. niepełnosprawne w stopniu umiarkowanym lub znacznym.REALIZOWANIE DZIAŁANIA (Instrumenty kompleksowej aktywizacji społeczno- zawodowej):- indywidualne spotkania z psychologiem i doradcą zawod. w celu utworzenia ścieżki reintegracji, podpis. zostanie umowa na wzór kontraktusocjalnego,- trening umiejętności społ.,- indywidualne poradnictwo psychologiczne/ psychospołeczne,- usługi rehabilitacyjne/ fizjoterapeutyczne,- wyjazdy na wydarzenia kulturalne,- indywid. poradnictwo prawne,- indywid. szkolenia zawodowe,- pośrednictwo pracy,- staże zawod.Formy wsparcia zostaną dopasowane do indywid. potrzeb uczestnika.REZULTATY:- W proj. 100% uczestn. nabędzie umiejętności społ., a min. 80% kompetencje/ kwalifikacje zawodowe. - Min. 34% uczestn. dokona postępu w procesie aktywizacji społecznej i zmniejszy dystans do zatrudnienia, a min. 12% ON i 25% pozostałych os. zagroż. wykluczeniem społ. podejmie zatrudnienie po zakończeniu udziału w proj. OBJECTIVE OF THE PROJECT: socio-professional reintegration, during the period IX.2019-XII.2020,-200os adults (120K,80M) passive profession, at risk of poverty or exclusion of people – benefit from social assistance benefits pursuant to the Act of 12 March 2004 on social assistance or eligible for social assistance – for poverty (100os) or persons with disabilities (100os, including at least 20 SAs in moderate or significant) riots in Wielkopolskie – through the implementation of individual reintegration paths include instruments of socio-professional activation. TARGET GROUP: 200 (120K,80M) persons inactive, at risk of poverty or social exclusion, inhabited by the KC in the area of Wielkopolska voivodship, which are:- persons benefiting from social assistance benefits pursuant to the Act of 12 March 2004 on social assistance or eligible for support for poverty assistance (i.e. fulfilling the poverty condition in accordance with Article 7 of the Act), and/or – persons with disabilities, including at least 10 % of the participants are disabled to a moderate or significant degree. REALISATION OF ACTION (Instruments for comprehensive socio-professional activation):- individual meetings with a psychologist and professional advisor. in order to create a path of reintegration, a contract will be signed.- training of social skills,- individual psychological/psychosocial counselling,- rehabilitation/physiotherapeutic services,- trips to cultural events,- individual. legal counselling,- individual. vocational training,- job placement,- internships profession. Support forms will be tailored to the needs of the participant.REZULTATY:- In the project. 100 % of participants will acquire skills in society and at least 80 % of professional competences/qualifications. — At least 34 % of participants will make progress in the process of social activation and reduce the distance to employment, and at least 12 % of the SA and 25 % of the remaining persons at risk of social exclusion will take up employment after the end of their participation in the project.
