Celem projektu jest zwiększenie dostępu do zintegrowanych usług społecznych odpowiadających na potrzeby społeczności lokalnej poprzez utworzenie Centrum Usług Społecznych w Gminie Czarnia, w którym wsparciem zostanie objętych 603 (303K, 300M) osób z terenu Gminy Czarnia (według przepisów KC) w okresie 1.01.2021-31.05.2023 r.Grupę docelową projektu stanowią:- pracownicy jednostek samorządu terytorialnego 8 pracowników CUS w Czarni (6K, 2M) objętych wsparciem w projekcie, w tym 6 osób w ramach kadry obecnie funkcjonujące GOPS i 2 osoby w ramach nowo utworzonych stanowisk pracy- mieszkańcy gminy, w której organizowane jest centrum usług społecznych 595 osób (297K, 298M) mieszkańców Gminy Czarnia, w tym 415 osób (207K, 208M) objętych wsparciem w postaci usług społecznych oraz 180 (90K, 90M) objętych wsparciem w ramach usług zdrowotnych.Zadania zaplanowane do realizacji to 4 kamienie milowe, na kt. składają się:I - Opracowanie Planu Wdrażania CUSII - Realizacja Planu Wdrażania CUS i świadczenie usług społecznychIII - Aktualizacja i realizacja Planu Wdrażania CUS i świadczenie usług społecznychIV - Wypracowane wnioski z realizacji Planu Wdrażania CUS i świadczenie usług społecznych.Rezultaty projektu zakładają m.in.:- rozwój i integrację systemu usług społecznych na poziomie lokalnym,- realizację Planu Wdrażania CUS,- opracowanie i realizację programu usług społecznych,- świadczenie usług społecznych dla 415 osób (207K, 208M) mieszkańców Gminy Czarnia,- świadczenie usług zdrowotnych dla 180 (90K, 90M) mieszkańców Gminy Czarnia,- podniesienie kompetencji i kwalifikacji 8 pracowników (6K, 2M) Gminy Czarnia.Zgodnie z koncepcją przyjętą w projekcie ustawy, planowane do utworzenia Centrum Usług Społecznych w Czarni będzie dążyło do zaspokajania potrzeb wspólnoty samorządowej w zakresie określonych usług społecznych oraz koordynacji tych usług. The aim of the project is to increase access to integrated social services responding to the needs of the local community by setting up a Social Services Centre in the Czarnia Municipality, which will support 603 (303K, 300M) people from the area of the Czarnia Commune (according to the provisions of the CC) in the period 1.01.2021-31.05.2023.The target group of the project is:- employees of local government units of 8 employees of the CUS in Czarni (6K, 2M) supported in the project, including 6 people within the staff currently functioning GOPS and 2 persons in the newly created jobs – residents of the municipality, where the centre of social services is organised 595 people (297K, 298M) residents of the municipality of Czarnia, including 415 people (207K, 208M) supported in the form of social services and 180 (90K, 90M) supported by health services.The tasks planned for implementation are 4 milestones, which consists of: I – Development of the CUSII Implementation Plan – Implementation of the CUS Implementation Plan and the provision of social servicesiii – Update and implementation of the CUS Implementation Plan and the provision of social servicesIV – Elaborated conclusions from the implementation of the CUS Implementation Plan and the provision of social services.The results of the project include:- the development and integration of the social services system at the local level,- implementation of the CUS Implementation Plan,- the development and implementation of the social services programme,- the provision of social services for 415 people (207K, 208M) residents of the Czarnia Municipality,- the provision of health services for 180 (90K, 90M) residents of the Czarnia Municipality,- the improvement of competences and qualifications of 8 employees (6K, 2M) of the Czarnia Municipality.In accordance with the concept adopted in the draft law, the planned establishment of the Social Services Centre in Czarni will strive to meet the needs of the local government community in the field of certain social services and coordinate these services. L’objectif du projet est d’améliorer l’accès aux services sociaux intégrés répondant aux besoins de la communauté locale par la mise en place d’un centre de services sociaux dans la municipalité de Czarnia, qui soutiendra 603 (303K, 300 millions) de la zone de la commune de Czarnia (selon les dispositions du CC) au cours de la période allant du 1.01.2021-31.05.2023.Le groupe cible du projet est:- les employés des collectivités locales de 8 employés du CUS à Czarni (6K, 2M) soutenu dans le projet, y compris 6 personnes au sein du personnel actuellement en service GOPS et 2 personnes dans les nouveaux emplois — résidents de la municipalité, où le centre des services sociaux est organisé 595 personnes (297K, 298 M) résidents de la municipalité de Czarnia, dont 415 personnes (207K, 208M) soutenues sous la forme de services sociaux et 180 (90K, 90M) soutenue par les services de santé.Les tâches prévues pour la mise en œuvre sont 4 étapes, qui comprend: I — Élaboration du plan de mise en œuvre du CUSII — Mise en œuvre du plan de mise en œuvre du CUS et prestation de services sociauxiii — Mise à jour et mise en œuvre du plan de mise en œuvre du CUS et de la prestation de services sociauxIV — Conclusions élaborées à partir de la mise en œuvre du plan de mise en œuvre du CUS et de la prestation de services sociaux.Les résultats du projet comprennent:- le développement et l’intégration du système de services sociaux au niveau local,- la mise en œuvre du plan de mise en œuvre du CUS,- le développement et la mise en œuvre du programme de services sociaux,- la fourniture de services sociaux à 415 personnes (207K, 208M) résidents de la municipalité de Czarnia,- la prestation de services de santé pour 180 (90K, 90M) résidents de la municipalité de Czarnia,- l’amélioration des compétences et des qualifications de 8 employés (6K, 2M) de la municipalité de Czarnia.Conformément au concept adopté dans le projet de loi, la création prévue du Centre des services sociaux à Czarni s’efforcera de répondre aux besoins de la collectivité locale dans le domaine de certains services sociaux et de coordonner ces services.
