Celem głównym projektu "Wszystko jest możliwe" realizowanym przez Gminny Ośrodek Pomocy Społecznej w Grabowcu jest integracja społeczna i zawodowa 40 osób (w tym 24 K i 16 M) z niepełnosprawnościami, zagrożonych wykluczeniem społecznym i ubóstwem z terenu gminy Grabowiec i Skierbieszów poprzez realizację kompleksowego programu integracji społecznej i zawodowej osób niepełnosprawnych do 31.07.2017 roku. Kluczowe rezultaty projektu: WSKAŹNIKI PRODUKTU - Liczba osób zagrożonych ubóstwem lub wykluczeniem społecznym objętych wsparciem w Programie- 40 osób - Liczba osób z niepełnosprawnościami objętych wsparciem w programie- 40 osób WSKAŹNIKI REZULTATU BEZPOŚREDNIEGO - Liczba osób zagrożonych ubóstwem lub wykluczeniem społecznym, które uzyskały kwalifikacje po opuszczeniu programu-28 osób - Liczba osób zagrożonych ubóstwem lub wykluczeniem społecznym poszukujących pracy po opuszczeniu programu- 23 osoby - Liczba osób zagrożonych ubóstwem lub wykluczeniem społecznym pracujących po opuszczeniu programu (łącznie z pracującymi na własny rachunek.)-10 osób Grupę docelową stanowią osoby wykluczone (w tym dotknięte ubóstwem) lub osoby zagrożone ubóstwem lub wykluczeniem społecznym z terenu woj. lubelskiego - zamieszkujące Gminę Grabowiec i Gminę Skierbieszów w tym osoby z niepełnosprawnościami (w tym z zaburzeniami psychicznymi)- 40 osób (24 K i 16 M), które nie pracują (są bezrobotne lub nieaktywne zawodowo). Co najmniej 50% uczestników projektu (20 osób) korzysta z Programu Operacyjnego Pomoc Żywnościowa 2014-2020. Zadania (4) realizowane w ramach Programu integracji społecznej i zawodowej osób niepełnosprawnych to: Diagnoza potrzeb, Praca socjalna, Aktywna integracja, Zasiłki i pomoc w naturze. Działania w ramach projektu będą adekwatne do zdiagnozowanych specyficznych potrzeb grupy docelowej m.in. poprzez zastosowanie indywidualnych planów działania wsparcia. (Polish)
