The main objective of the project is to activate the socio-occupational – occupational activity of 86 persons (58K, 28M) at risk of poverty or social exclusion, living in accordance with the Wielkopolska Voivodeship of Wielkopolska Voivodeship, through an active inclusion programme and participation in comprehensive social support up to 29.06.2019.Under the project the following indicators have been attained by the project: These will be persons benefiting from social assistance in accordance with the Act of 12.03.2004 on social aid or eligible for social assistance.The project will include persons fulfilling at least 1 conditions at risk of poverty or social exclusion:- passive persons of the Polish Labour Party – 66 UP (40 K, 26 M),- poor persons working for a degree of poverty (18 K, 2 M), – persons with disabilities – 9 PP, 5 % of the total number of persons not employed in the labour force (18 K, 2 M) – persons with disabilities – 4 of the Polish People’s Republic of Poland (40 K, 26 M). Celem głównym proj jest aktywizacja społeczno - zawodowa 86 osób (58K, 28M) zagrożonych ubóstwem lub wykluczeniem społecznym, zamieszkujących w myśl KC woj. wielkopolskie w Gminie Miasteczko Krajeńskie , poprzez programie aktywnej integracji i udział w kompleksowym wsparciu do 29.06.2019.W ramach projektu założono osiągnięcie następujących wskaźników:• Liczba osób zagrożonych ubóstwem lub wykluczeniem społecznym objętych wsparciem w programie – 86• Liczba osób z niepełnosprawnościami objętych wsparciem w programie - 9• Liczba osób zagrożonych ubóstwem lub wykluczeniem społecznym, które uzyskały kwalifikacje po opuszczeniu programu – 6• Liczba osób zagrożonych ubóstwem lub wykluczeniem społecznym poszukujących pracy po opuszczeniu programu – 20• Liczba osób zagrożonych ubóstwem lub wykluczeniem społecznym pracujących po opuszczeniu programu (łącznie z pracującymi na własny rachunek) – 10.Grupę docelową stanowi 86 osób (58 K, 28 M) zamieszkałych w Gminie Miasteczko Krajeńskie, zagrożonych ubóstwem lub wykluczeniem społ. Będą to osoby korzystające ze świadczeń z pomocy społ. zg. z ustawą z 12.03.2004 r. o pomocy społ. lub kwalifikujące się do objęcia wsparciem pomocy społ.Do projektu zostaną przyjęte osoby spełniające min. 1 przesłankę zagrożenia ubóstwem lub wykluczeniem społecznym:- os bierne ZAW - 66 UP (40 K, 26 M),- ubogie os pracujące 20 UP (18 K, 2 M),- os niepełnosprawne - 9 UP (5K, 4M), w tym 4 o stopniu umiarkowanym, 4 o znacznym stopniu niepełnospr., osoby z niepełnospr. intelekt., osoby z niepełnospr. sprzężonymi ,- os korzystające z POPŻ - 61 os. W ramach projektu zostaną zrealizowane takie zadania, jak:1. Aktywizacja społeczna2. Aktywizacja zdrowotna3. Aktywizacja edukacyjna4. Aktywizacja zawodowa5. Działania o charakterze środowiskowym (Polish)
