Projekt polega na stworzeniu dziennego Brackiego Domu dla Seniorów z 12 nowymi miejscami zdeinstytucjonalizowanej opieki nad osobami niesamodzielnymi zlokalizowanego w budynku Przychodni Brackiej Staszic w Katowicach. Będzie skierowany do osób starszych w wieku powyżej 65 roku życia, które w skali Barthel uzyskały wynik 40-80 punktów – osoby te mają problem z wykonywaniem co najmniej jednej czynności życia codziennego. Dzięki realizacji projektu możliwa będzie poprawa stanu zdrowia osób niesamodzielnych, biorących w nim udział oraz przywrócenie tych osób do życia w lokalnej społeczności. Wsparcie w ramach projektu wykracza poza gwarantowane świadczenia opieki zdrowotnej obejmując pacjentów kompleksową opieką i kierowane będzie do osób niesamodzielnych, wymagających opieki, przebywających w domach.
