The project “Smartopieka 2.0” involves comprehensive support for people at risk of social exclusion or marginalisation due primarily to old age and is complementary to the existing system because it includes elderly/independent persons, their carers as well as a group of actual carers. As part of the project, 100 people – inhabitants of the Silesian Voivodeship – will be covered by high-quality care services in the place of residence. Care services will be provided on the basis of modern IT technologies enabling supervision over the work of caregivers and collecting data on life parameters of patients. Participants of the project will also receive support provided by a geriatric nurse and a geriatric doctor, as well as support from an independent and senior caregiver. Equipment rental (IT spheres, laptops) will be launched. Support (e.g. training, advice) will also include 200 actual carers. (English)

    Projekt ”Smartopieka 2.0” zakłada kompleksowe wsparcie osób zagrożonych wykluczeniem społecznym lub marginalizacją ze względu przede wszystkim na podeszły wiek i jest komplementarny do istniejącego systemu ponieważ obejmuje osoby starsze/niesamodzielne, ich opiekunów, jak również grupę opiekunów faktycznych. W ramach projektu 100 osób - mieszkańców woj. śląskiego - objętych zostanie usługami opiekuńczymi wysokiej jakości w miejscu zamieszkania. Usługi opiekuńcze świadczone będą w oparciu o nowoczesne technologie informatyczne umożliwiające nadzór nad pracą opiekunek oraz zbieranie danych o parametrach życiowych podopiecznych. Uczestnicy projektu dodatkowo objęci zostaną wsparciem świadczonym przez pielęgniarkę geriatryczną oraz lekarza geriatrę, a także wsparciem doradcy osoby niesamodzielnej oraz starszych opiekunów. Uruchomiona zostanie wypożyczalnia sprzętu (kule IT, laptopy). Wsparciem (m.in. szkolenia, doradztwo) objętych zostanie również 200 opiekunów faktycznych. (Polish)
