Projekt stanowi II etap rozwoju usług społecznych na trenie Zawiercia, projekt w okresie od 2019-09-01 do 2021-08-31 r. obejmie rozwój i upowszechnienie dostępności do usług świadczonych w lokalnej społeczności, w szczególności na obszarach objętych rewitalizacją. Działania w projekcie dotyczą: mieszkalnictwa wspomaganego dla osób z niepełnosprawnościami, usług opiekuńczych dla osób starszych i niesamodzielnych, usług wsparcia rodziny. Projektem zostanie objętych 119 osób, w tym 54 M i 65 K znajdujących się w niekorzystnej sytuacji społecznej ze względu na posiadane deficyty, osoby zagrożone ubóstwem lub wykluczeniem społecznym, w tym osoby z niepełnosprawnościami, osoby niesamodzielne, dzieci z rodzin przeżywających trudności opiekuńczo-wychowawcze.
