Project implemented in the period 01.09.2018-31.12.2019 by nursing and caring SERVICES SERVICES in partnership with NEUROREH COMPANY WITH RESTRICTION. The project implements the following types of project:1) Development of deinstitutionalised forms of medical care for the elderly, including disability, in the field of, among others: supporting activities or the creation of new medical facilities, including long-term home medical care, increasing access to services by ensuring the necessary transport; 2) Development of measures to improve the health of older people, including disability, including: physical, psychiatric and speech therapy. In addition, the project will provide social services. This project is addressed to persons or families at risk of poverty or social exclusion, the environment of people at risk of poverty or social exclusion, and staff providing health services living in the area of the voivodship – in particular Mr Opole and Krapkowice. 8 patients (5K and 3M), 10(8K; 2M) staff of services providing health and social services – rehabilitants (2K); 1M), physician-surgeon (0K; 1M), nurses (3K; 0M) and babysitters (3K; 0M).As part of this project, the provision of health services for the elderly, including disability, will be ensured. Projekt realizowany w okresie 01.09.2018-31.12.2019 przez USŁUGI PIELĘGNIARSKIE I OPIEKUŃCZE MAŁGORZATA TESARZ w partnerstwie z NEUROREHA SPÓŁKA Z OGRANICZONĄ ODPOWIEDZIALNOŚCIĄ. Projekt realizuje następujące typy projektu:1) Rozwój zdeinstytucjonalizowanych form opieki medycznej nad osobami starszymi, w tym z niepełnosprawnościami, w zakresie m.in.: wsparcia działalności lub tworzenia nowych miejsc opieki medycznej, w tym długoterminowej medycznej opieki domowej, zwiększenia dostępu do usług poprzez zapewnienie niezbędnego transportu; 2) Rozwój działań wpływających na poprawę stanu zdrowia osób starszych, w tym z niepełnosprawnościami, w tym : rehabilitacja ruchowa, psychiatryczna i logopedyczna. Dodatkowo w projekcie świadczone będą usługi społeczne. Niniejszy projekt skierowany jest do osób lub rodzin zagrożonych ubóstwem lub wykluczeniem społecznym, otoczenia osób zagrożonych ubóstwem lub wykluczeniem społecznym oraz personelu służb świadczących usługi zdrowotne zamieszkujących teren woj.opolskiego- w szczególności m.Opola oraz Krapkowic. Wsparciem zostanie objętych 8 pacjentów(5K i 3M), 10(8K;2M) osób stanowiących personel służb świadczących usługi zdrowotne i społeczne - rehabilitanci (2K; 1M), lekarz -chirurg(0K; 1M), pielęgniarki(3K; 0M) oraz opiekunki (3K; 0M).W ramach niniejszego projektu zostanie zapewnione świadczenie usług zdrowotnych nad osobami starszymi, w tym z niepełnosprawnościami.
