The aim of the project is to increase the chances of social and occupational activation of people excluded and at risk of social exclusion and preventing the phenomenon of exclusion of the population and poverty by including support under the active integration instruments of 120 people at risk of poverty and the population benefiting from the benefits of MOPS services in Płock.The implementation of the project will take into account the empowerment-participants’s need in the process of planning activities, which will allow for matching the support to be used to real efficiency. The project complies with the principle of equal opportunities K and M, non-discrimination of ON, with the principle of sustainable development and will use validated final products developed in innovative projects at PIW EQUAL and POKL. Cel projektu to zwiększenie szans na aktywizację społeczną i zawodową os. wykluczonych i zagrożonych wykluczeniem społecznym oraz zapobieganie zjawisku wykluczenia społ.i ubóstwa poprzez objęcie wsparciem w ramach instrumentów aktywnej integracji 120 os.zagrożonych ubóstwem i wykl.społ. korzystających ze świadczeń MOPS w Płocku.Realizacja projektu będzie uwzględniała zasadę empowerment-partycypowanie uczestników w procesie planowania działań co pozwoli na dopasowanie wsparcia adekwatnego do rzeczywistych potrzeb co zwiększy efektywność wykorzystanych środków.Punktem wyjścia będzie diagnoza potrzeb uczestników projektu na podstawie której powstanie ścieżka reintegracji stworzona indywidualnie dla każdej osoby z uwzględnieniem diagnozy,sytuacji problemowej lub zagrożenia sytuacją problemową,zasobów,potencjału,predyspozycji,potrzeb.Zaplanowane wsparcie uwzględnia usługi aktywnej integracji o charakterze społecznym,edukacyjnym,zawodowym i zdrowotnym które będą skupiały się min. na:nabyciu/przywróceniu/wzmocnieniu komp.społ.wyposażeniu w kwalifikacje zaw.kompetencje niezbędne na rynku pracy,wyeliminowaniu barier zdrowotnych,zintegrowane poradnictwo specjalistyczne. Osoby bezrobotne zakwalifikowane do III profilu pomocy będą objęte wsparciem realizowanym na podstawie Programu Aktywizacja i Integracja- 33% grupy docelowej (40 osób). W ramach PAI osoby bezrobotne skorzystają z wsparcia o charakterze zawodowym-prace społecznie użyteczne oraz integracyjno-społecznym-grupowe warsztaty z psychologiem i coachem. Poprzez realizację projektu nastąpi min.wzrost poziomu włączenia społecznego u 120 uczestników projektu,wzrost efektywności społeczno-zatrudnieniowej u min.56% uczestników( 68os.)w tym wzrost poziomu efektywności zatrudnieniowej u min.22% uczestników (27osób).
