Celem głównym projektu jest wzrost dostępności usług zdrowotnych świadczonych wobec osób niesamodzielnych w formie zdeinstytucjonalizowanej zamieszkałych w rozumieniu przepisów Kodeksu Cywilnego na terenie ŁOM w okresie pomiędzy wrześniem 2018 r. a kwietniem 2020 r. W projekcie przewidziano objęcie grupy docelowej złożonej z 36 pacjentów (w tym 20 kobiet i 16 mężczyzn) usługami zdrowotnymi w postaci domowej pielęgniarskiej opieki długoterminowej realizowanej w domu pacjenta wraz z rehabilitacją, na którą składa się ocena medyczna lekarza rehabilitanta, ustalenie indywidualnych zabiegów rehabilitacyjnych dla pacjenta, wykonywanych przez zespół rehabilitantów i masażystkę oraz wsparciem psychologiczno-terapeutycznym dla opiekunów faktycznych. Rezultat projektu przyczyni się do złagodzenia problemów w ww. zakresie w regionie ŁOM. (Polish)
