The project “Give hope – support and development” in accordance with the initiated partnership between the Association of AD REM and OPS AŁ and MOPS ZG assumes the extension of the NOTA BENE Club’s offer (daily support facility) and the creation of a new facility – Club OBUDZ JUTRO in Zgirz. Clubs address their activities to a group of 100 people (60Dz and 40Ch) children and school youth of the municipality of AŁ and Municipality of ZG, including 6 people with light/moderate disabilities. These people come from families diagnosed by JST (schools, OPS, MOPS) as dysfunctional, multi-problem and at risk of social marginalisation. The project involves conducting systematic care, development and educational activities, which respond to the diagnosed needs of children and adolescents. The tasks will be carried out by: 1. activities of NOTA BENE Clubs and OBUDY JUTRO, organising thematic activities for 5-6 days in a week; 2.implementation of an alternative climbing programme “On the way to the summit”;3.programmes to help children through family work;4.professional support(Biofeedback and HEG therapy,psychologist, psychologist, logopeda, lawyer)5.Streetworking. Each of the project participants will be diagnosed during individual work with a psychologist/IWR specialist and will actively participate in the development of an individual development path, in which the most appropriate types of activities for a given participant will be identified, affecting their further personal and social development.Psychologist, animator and trainers will be responsible for the proper course of implementation of individual participants’ path as well as monitoring, evaluation and correction of findings. The implementation of these actions will ensure, inter alia, the development of learning skills, the acquisition of social and civic competences, the development of initiative and entrepreneurship. Projekt "Podaruj nadzieję - wsparcie i rozwój" zgodnie z zainicjowanym partnerstwem pomiędzy Stowarzyszeniem AD REM a OPS AŁ i MOPS ZG zakłada rozszerzenie oferty Klubu NOTA BENE (placówki wsparcia dziennego) i utworzenie nowej placówki- Klubu OBUDŹ JUTRO w Zgierzu. Kluby adresują swoje działania do grupy 100 osób( 60Dz i 40Ch) dzieci i młodzieży szkolnej Gminy AŁ i Gminy Miasta ZG, w tym 6 osób z upośledzeniem lekkim/umiarkowanym. Osoby te pochodzą z rodzin zdiagnozowanych przez JST (szkoły, OPS, MOPS) jako dysfunkcyjne, wieloproblemowe i zagrożone marginalizacją społeczną. Projekt zakłada prowadzenie systematycznych zajęć opiekuńczych, rozwojowych oraz edukacyjnych, będących odpowiedzią na zdiagnozowane potrzeby dzieci i młodzieży. Zadania realizować będzie poprzez: 1.działania Klubów NOTA BENE i OBUDŹ JUTRO, organizujących zajęcia tematyczne przez5-6 dni w tyg; 2.realizację alternatywnego programu wspinaczkowego „W drodze na szczyt”;3.programy pomocy dzieciom poprzez pracę z rodziną;4.prowadzenie wsparcia specjalistycznego(terapia Biofeedback i HEG,psycholog,psychiatra,logopeda,prawnik)5.Streetworking. Każdy z uczestników projektu zostanie zdiagnozowany podczas indywidualnej pracy z psychologiem/specjalistą IWR oraz będzie aktywnie uczestniczył przy wypracowywaniu indywidualnej ścieżki rozwoju, w której wskazane zostaną najodpowiedniejsze dla danego uczestnika typy zajęć, mających wpływ na jego dalszy osobisty i społeczny rozwój.Psycholog,animator i trenerzy odpowiadać będą za właściwy przebieg realizacji ścieżki poszczególnych uczestników oraz monitoring, ewaluację i korygowanie ustaleń. Realizacja w/w działań zagwarantuje uczestnikom między innymi rozwój umiejętności uczenia się, nabycie kompetencji społecznych i obywatelskich, rozwinie inicjatywność i przedsiębiorczość. (Polish)
