| Property / summary: The project involves the creation of 20 places in the daily subsistence buildings of the municipality of Aleksandrów Kujawski in the town of straws. In the framework of the DDP operation, care and support services will be supported 42. This has been done by other functions so far, so it is essential to work on adaptation, where the building will be adapted for people with disabilities (zd 1). In order to be able to take place there will be rehabilitation equipment and physical and mental improvement workshops with equipment for rehab. as well as equipment for a dignified and active time (zd 1). In rural municipalities, a poor transport network is a barrier to access to social services, and everyone will be able to benefit from the feeder service to the DDP. The offer will be tailored to the individual needs of the participants for which different forms of individual and group support will be prepared. All participants will be offered two meals and, in addition, they will be able to benefit from the equipped kitchen (d 2).In addition, the support for the provision of dependants will receive 42 carers (3) in order to ensure better care and integration in the local community even after the completion of the participation in the project. Support for factual carers will take the forms of: participation in workshops, group guidance, legal advice and psycho-social support. Projekt zakłada utworzenie 20 miejsc w Dziennym Domu Pobytu w Gminie Aleksandrów Kujawski w miejscowości Słomkowo. W ramach funkcjonowania DDP usługami opiekuńczymi oraz wspierającymi objęte zostaną 42 os. niesamodzielne. Pomieszczenia przeznaczone na ten cel do tej pory pełniły inne funkcje dlatego też niezbędnym jest dokonanie prac adaptacyjnych, w ramach których budynek zostanie przystosowany dla osób niepełnosprawnych (zad 1). Aby mogły tam odbywać się zajęcia rehabilitacyjne oraz warsztaty poprawiające funkcjonowanie pod względem fizycznym jak i psychicznym zakupiony zostanie sprzęt rehab. a także elementy wyposażenia umożliwiające godne i aktywne spędzenie czasu (zad 1). W gminach wiejskich barierą dostępu do usług społecznych jest słaba sieć transportowa dlatego też wszyscy chętni będą mogli skorzystać z usługi dowozu do DDP. Oferta zostanie dostosowana do indywidualnych potrzeb uczestników dla których zostaną przygotowane zróżnicowane formy wsparcia indywidualnego jak i grupowego. Wszyscy uczestnicy otrzymają dwa posiłki a dodatkowo na bieżąco będą mogli skorzystać z wyposażonej kuchni (zad 2).Dodatkowo wsparcie w zakresie opieki nad osobą niesamodzielną otrzyma 42 opiekunów faktycznych (zad3) po to by zapewnić lepszą opiekę i integrację ze społecznością lokalną również po zakończeniu udziału w projekcie. Wsparcie dla opiekunów faktycznych przyjmie formy: udział w warsztatach, poradnictwo grupowe, poradnictwo prawne i wsparcie psychologiczne.Odbiorcami wsparcia są mieszkańcy gminy Aleksandrów Kujawski – gdzie do tej pory nie funkcjonuje żadna instytucja z tego typu ofertą.Po zakończeniu proj. zapewniona zostanie jego trwałość co najmniej w okresie odpowiadającym okresowi realizacji projektu.Gł. wskaźniki rezultatu i produktu: Liczba wspartych w programie miejsc świadczenia usług społ. – 21; Liczba osób zagrożonych ubóstwem lub wykluczeniem społ. objętych usługami społ. – 42, Liczba opiekunów faktycznych/osób z otoczenia osób niesam. objętych wsparciem – 42.
