The project “Gminny Model of Care Services” (GMUO) aims to facilitate access to affordable, sustainable and high quality care services provided to 95 persons (59 K and 36 M) of which 65 independent(ON)- (K-39, M-26) and 30 factual guardians (OF) (20 K and 10 M), residents of the municipality Nowe. The diagnosis shows that there has been insufficient support to date in this area: it included a small number of people, their quality did not satisfy the basic expectations of the ON. As part of the GMUO, we plan to implement a number of activities, involve both help specialists, representatives of local NGOs, as well as involving local community residents and ON family. The main objective of the project is to improve the quality and accessibility of environmental care and care services provided at home for dependents from the area of Nowe commune through the implementation of GMUO in the years 2018-2020. In order to achieve the objective, the performers envisaged the following tasks for 95 people (59 women, 36 men): educational support for OF in the form of lectures, setting up a self-help group for OF, specialised care services for ON (psychologist, psychiatrist, nurse, rehabilitant), support of the assistant of disabled persons and environmental carers, involvement of the local community, through the inclusion of senior volunteering and neighbourhood assistance, facilitating access to support and care equipment through the creation of a rental facility. The project developers assume that they will qualitatively change this area of support within the commune, and the model tested under the project will be another permanent tool permanently included in the scope of local social support. An additional advantage will be the recommendation of the model as an effective tool dedicated to similar demographic communities. (English)

    Projekt „Gminny Model Usług Opiekuńczych” (GMUO) ma na celu ułatwienie dostępu do przystępnych cenowo, trwałych oraz wysokiej jakości usług opiekuńczych świadczonych 95 osobom (59 K i 36 M) w tym 65 niesamodzielnym(ON)- (K-39, M- 26) oraz 30 opiekunów faktycznych (OF) (20 K i 10 M), mieszkańców gminy Nowe. Z przeprowadzonej diagnozy wynika, że dotychczasowe wsparcie w tym obszarze było niewystarczające: obejmowało niewielką ilość osób, ich jakość nie zaspakajała podstawowych oczekiwań ON. W ramach GMUO planujemy wdrożyć szereg działań, zaangażować zarówno specjalistów pomocy, przedstawicieli lokalnych NGO, jak i włączyć do działania mieszkańców lokalnej społeczności oraz rodziny ON. Celem głównym projektu jest podniesienie jakości i zwiększenie dostępności środowiskowych usług opiekuńczo-pielęgnacyjnych świadczonych w domu dla osób zależnych z terenu gminy Nowe poprzez wdrożenie GMUO w latach 2018- 2020. Dla osiągnięcia celu realizatorzy przewidzieli następujące zadania dla 95 osób (59 kobiet, 36 mężczyzn): wsparcie edukacyjne dla OF w formie wykładów, utworzenie grupy samopomocowej dla OF, specjalistyczne usługi opiekuńcze dla ON (psycholog, psychiatra, pielęgniarka, rehabilitant), wsparcie asystenta osoby niepełnosprawnej i opiekunów środowiskowych, zaangażowanie społeczności lokalnej, poprzez włączenie wolontariatu seniorów oraz pomocy sąsiedzkiej, ułatwienie dostępu do sprzętu wspomagającego i pielęgnacyjnego poprzez utworzenie wypożyczalni. Realizatorzy projektując działania zakładają, że zmienią one jakościowo ten obszar wsparcia na terenie gminy, a przetestowany w ramach projektu model będzie kolejnym trwałym narzędziem na stałe włączonym w zakres lokalnego wsparcia społecznego. Dodatkowym atutem będzie rekomendacja modelu jako skuteczne narzędzie dedykowane podobnym demograficznie gminom. (Polish)
