Celem projektu jest zwiększenie dostępu do usług społecznych i opiekuńczych w Gminie Miasto Wąbrzeźno woj. kujawsko - pomorskie na obszarze rewitalizowanym w okresie od 01.07.2020 do 30.09 2021 dla 55 uczestników (40 K, 15 M) z grup defaworyzowanych, które są w najtrudniejszej sytuacji życiowej.Projekt przewiduje realizację kompleksowych usług społecznych realizowanych w środowisku lokalnym w tym:- Utworzenie Klubu Samopomocy, który świadczyć będzie usługi informacyjne, szkoleniowe i doradcze prawne, lekarza geriatry i psychologa dla uczestników 4 grup samopomocowych dla osób starszych - seniorów, rodziców samotnie wychowujących dzieci, i 2 równoległych grup osób przewlekle chorych, prowadzenie wypożyczalni sprzętu rehabilitacyjnego, usługi rehabilitacji, promocja i organizowanie wolontariatu w formie "banku czasu" czyli świadczenia usług wzajemnych, organizowanie imprez i wyjazdów integracyjnych. - Praca socjalna z uczestnikami;- Praca ciągła grup samopomocowych;- Warsztaty aktywizujące i podnoszące kompetencje uczestników Planowane efekty projektu:PRODUKT Liczba osób zagrożonych ubóstwem lub wykluczeniem społecznym objętych usługami społecznymi świadczonymi w interesie ogólnym w programie - 55 osóbProjekt wpisuje się w zakresie celów i rezultatów w Strategię Rozwoju Społeczno-Gospodarczego Powiatu Wąbrzeskiego i Program Rewitalizacji Miasta Wąbrzeźno na lata 2016-2023.Bez dodatkowego wsparcia z Unii Europejskiej zaplanowanych działań nie można by było zrealizować.
