Uchwałą nr 49/2017/XIV),b) długoterminową med. opiekę domową nad osobą niesamodzielną, w tym pielęgniarską opiekę długoterminową,c) pomoc dla opiekunów, w szczególności członków rodzin w postaci wsparcia psychologicznego i szkoleń w zakresie opieki med. nad osobami niesamodzielnymi,d) wdrożenie na poziomie lokalnym rozwiązań z zakresu koordynacji realizacji usług med. kierowanych do osób niesamodzielnych.Projekt będzie realizowany zgodnie z Modelem Centrum Koordynacji Opieki Medycznej dla osób niesamodzielnych i zapewnia dostęp do świadczonej oferty tj.
