SAMODZIELNY PUBLICZNY ZAKŁAD OPIEKI ZDROWOTNEJ W SZCZERCOWIE

Celem głównym projektu jest poprawa dostępności do usług zdrowotnych poprzez realizację programu indywidualnej opieki pielęgniarki środowiskowej w warunkach domowych dla 25 (13K i 12M) mieszkańców gminy Szczerców. Rezultatem projektu będzie wzrost liczby wspartych w programie miejsc świadczenia zdeinstytucjonalizowanych usług zdrowotnych. Osiągnięte to zostanie dzięki realizacji następujących zadań: 1. Kwalifikacja do projektu. 2. Objęcie 25 pacjentów opieką pielęgniarską oraz rehabilitacyjną. 3. Wsparcie dla opiekunów faktycznych osób niesamodzielnych. Projekt skierowany będzie do 25 osób w wieku 50+ zamieszkujących Gminę Szczerców, które zostaną objęte wsparciem w postaci indywidualnej opieki pielęgniarskiej i rehabilitacji w warunkach domowych oraz ich rodzin objętych wsparciem w postaci szkoleń z zakresu opieki nad osobami zależnymi (33 osoby – 17K i 16M). Łącznie z różnych form wsparcia skorzysta 58 os (30K i 28M). Wskaźniki dla projektu: • Liczba osób zagrożonych ubóstwem lub wykluczeniem społecznym objętych usługami zdrowotnymi świadczonymi w interesie ogólnym w programie – 25; • Ilość rodzin niesamodzielnych wsparta przez szkolenia – 25; • Liczba wspartych w programie miejsc świadczenia usług zdrowotnych – 8 • Liczba osób z otoczenia objęcia wsparciem w projekcie – 33 • Liczba wspartych w programie miejsc świadczenia usług zdrowotnych istniejących po zakończeniu projektu – 7 (Polish)

The main aim of the project is to improve accessibility to health services by implementing a programme of individual residential care services for 25 (13K and 12M) of local residents. The project will result in an increase in the number of institutionalised health services supported in the programme. This will be achieved through the following tasks: 1. Qualification for the project. 2. The inclusion of 25 patients in nursing and rehabilitation. 3. Support for carers of genuine dependent persons. The project will target up to 25 people aged 50 + living in the Municipality of Szczerwców, who will be supported by personal nursing and home rehabilitation care and their families supported by the provision of care training (33 persons 17K and 16M). In total, 58 os (30K and 28M) will benefit from the various forms of support. Indicators for the project: • Number of people at risk of poverty or social exclusion covered by public health services of general interest in the programme — 25; • The number of dependent families supported by the training — 25; • Number of health service places supported in the programme — 8• Number of people in the environment supported project — 33• Number of health service sites supported in the programme — 7 (English)
