Celem projektu jest zwiększenie liczby miejsc świadczenia usług opiekuńczych poprzez utworzenie 18 miejsc świadczenia środowiskowych usług opiekuńczych (niestacjonarnych) na terenie powiatu namysłowskiego. Projekt odpowiada na narastający problem starzenia się mieszkańców i niedostateczny poziom wsparcia opiekuńczego na terenie w/w powiatu. Powyższy cel zrealizowany zostanie w okresie 01.08.2017 r. - 31.12.2019 r. Powyższy cel zrealizowany zostanie poprzez 4 zadania: Zadanie 1. Organizacja środowiskowego centrum świadczenia usług opiekuńczych i specjalistycznych usług opiekuńczych w miejscu zamieszkania osób niesamodzielnych; Zadanie 2. Świadczenie usług opiekuńczych i specjalistycznych usług opiekuńczych dla osób niesamodzielnych w ich miejscu zamieszkania; Zadanie 3. Usługi wspierające pracę opiekunek i samodzielne funkcjonowania osób niesamodzielnych. Zadanie 4. Wsparcie opiekunów faktycznych. Projekt realizuje typ projektów podlegających dofinansowaniu 1a) i 1e), 3a) i 3b) oraz 4. Grupa docelowa: Projekt skierowany jest do dwóch grup zamieszkałych w powiecie namysłowskim: 1. 70 osób zagrożonych ubóstwem lub wykluczeniem społecznym - 70 osób niesamodzielnych, które będą objęte niestacjonarnymi usługami opiekuńczymi (usługi w miejscu zamieszkania danej osoby). 2. Otoczenie osób zagrożonych ubóstwem lub wykluczeniem społecznym, - 20 faktycznych opiekunów osób niesamodzielnych, którzy otrzymają wsparcie podnoszące ich kompetencje/umiejętności opiekuńcze, w tym także poradnictwo w zakresie ich uprawnień. Główne wskaźniki projektu: 1. Liczba wspartych w programie miejsc świadczenia środowiskowych usług społecznych istniejących po zakończeniu projektu – wartość docelowa: 18; 2. Odsetek opiekunów faktycznych, którzy wysoko ocenią wzrost swoich kompetencji opiekuńczych i wsparcia w procesie opieki nad ON – wartość docelowa: 80 %. 3. Liczba osób niesamodzielnych objętych usługami opiekuńczymi: 70 osób. (Polish)

The aim of the project is to increase the number of care facilities provided by the creation of 18 sites for the provision of community-based care services (ambulant) in the area of the district of reflection. The project responds to the growing problem of population ageing and insufficient care support in the district. This target will be achieved in the period 01.08.2017-31.12.2019. this target will be achieved through 4 tasks: Target 1. An environmental centre for the provision of care services and specialist care services at the dwelling of dependent persons; Target 2. Provision of care services and specialist care services for dependent persons in their place of residence; Target 3. Services supporting the work of carers and independent living of dependent persons. Target 4. Support for factual carers. The project shall implement the types of projects eligible for funding 1a) and 1e), 3a) and 3b) and 4. Audience: The project is addressed to two groups resident in the following districts: 1. 70 people at risk of poverty or social exclusion — 70 non-dependent persons who will be covered by non-stationary care services (services in the place of residence of the person). 2. The environment of people at risk of poverty or social exclusion, — 20 vulnerable carers who receive support to improve their competences/skills for carers, including guidance on their entitlements. Main project indicators: 1. Number of social services existing at the end of the project supported in the scheme — target: 18; 2. The share of carers who will highly assess the increase in caring responsibilities and support in the process of caring for ON — Target: 80 %. 3. The number of non-dependent persons covered by the provision of care services: 70 people. (English)
