Projekt obejmuje: -usługi opieki zastępczej, umożliwiające czasowy całodob pobyt osoby niesamodz w Ośrodku lub w miejscu zamieszk. w przypadku okres. braku możliwości sprawowania opieki przez jej opiekuna nieformalnego, -wsparcie edukac-doradcze opiekunów, obejmujące m.in. grupy wsparcia, poradnictwo, szkolenia i praktyki opiek, pomoc w zarządzaniu opieką, - zwiększanie dostępu opiekunów do informacji o różnych systemach wsparcia, -konferencje adresowane do kadr służby zdrowia i pomocy społ, -prowadzenie wypożyczalni sprzętu pielęgnac, rehabilitac. i med. wraz z nauką obsługi sprzętu i doradztwem w zakresie jego wykorzystania. Projekt realizowany jest przez Gminę Miasta Tarnowa w partnerstwie z 6 gminami Subregionu Tarnowskiego i Stowarzyszeniem eMagra. (Polish)

The project is being implemented by the Municipality of Tarnów, in partnership with the 6 municipalities of the Tarnowski Subregion and the eMagra Association. (English)
