Projekt obejmuje utworzenie Domu Dziennej Opieki Medycznej DDOM, wyodrębnioną strukt. cześć podmiotu leczn., posiadającego umowę o udzielanie św. opieki zdrow. z MOW NFZ, w którym zapewni się op. med. w warunkach zbliżonych do domowych os. zależnym niesamodzielnym, w tym os. starszymi powyżej 65 r.ż których stan zdrowia nie pozwala na pozostawanie wyłącznie pod opieką POZ i AOS, a jednocześnie nie wymagają całodobowego nadzoru lekarskiego i piel. w trybie stacjonarnym10 miejsc pobytu dziennego. W DDOM zapewniona zostanie op. personelu med.: piel., lekarska lekarz ubez. zdrowotnego, spec. rehab. med., spec. geriatra, fizjo., opiekuna medycznego, terapeuty zajęciowego, psychologa, logopedy, wyżywienie, bezpłatny dojazdtransport w razie potrzeby, edukacja rodziny i opiekunów UP do kontynuacji opieki.
