Projekt „Wsparcie osób niesamodzielnych szansą na skuteczne usługi zdrowotne” ma na celu wsparcie działań na rzecz deinstytucjonalizacji usług medycznych osób zagrożonych wykluczeniem społecznych/wykluczonych lub niesamodzielnych. Usługi zdrowotne będą świadczone w społeczności lokalnej. Wsparcie ukierunkowane będzie na wdrożenie lokalnych rozwiązań z zakresu koordynacji usług medycznych dla 110 uczestników projektu (60K i 50M), a także ich rehabilitacji domowej prowadzonej przez wykwalifikowany personel, gwarantującej tym osobom warunki na niezależne życie w środowisku lokalnym. Ważne będzie również wsparcie psychologiczne dla opiekunów faktycznych (55 os. w tym 30K i 25 M) opiekujących się osobami niesamodzielnymi. Świadczeniem usług z zakresu rehabilitacji medycznej wnioskodawca obejmie min 50% uczestników projektu. Projektem w okresie od 1.09.2019r. do 31.08.2020r. zostanie objętych 165 osób, w tym 110 osób niesamodzielnych. (Polish)

The project “Support for people who do not stand alone as an opportunity for effective health services” aims at supporting measures aimed at deinstitutionalisation of the medical services of people at risk of social exclusion/excluded or independent. Health services will be provided in the local community. The support will focus on the implementation of local medical coordination solutions for the 110 project participants (60K and 50M), as well as their home rehabilitation carried out by qualified staff, ensuring conditions for independent living in the local environment. It will also be important to provide psychological support for factual carers (55 of whom 30 K and 25 M) caring for dependants. The applicant will receive 50 % of the project participants in the provision of medical rehabilitation services. The project will cover 165 people in the period from 1.09.2019 to 31.08.2020, including 110 dependent persons. (English)
