The project was developed with 1 Partner — Specialist 24 — “KARDIOLOGIA” Sp. z o.o. Sp. komandytowa. The project covered all counties of the Opolskie Voivodeship. The need to implement the project stems from the demographic situation in the Opolskie Voivodeship, i.e. an increase in the number of people in Poland. 60 years of age, including at risk of poverty or social exclusion and/or with disabilities with elderly diseases, and they do not have access to quality health services that improve their health, functionality and reduce premature mortality. The tasks planned in the project are aimed at eliminating the above barriers. The project foresees the following tasks: telemedicine services for 150 elderly people (project participants) at risk of poverty or social exclusion (non-self-employed). The project will contribute to the main objective of enabling access to quality health services for older people at risk of social exclusion, thereby giving them the opportunity to extend and improve their quality of life. Projekt został opracowany z 1 Partnerem - Specjalistką 24 - "KARDIOLOGIA" Sp. z o.o. Sp. komandytowa. Projektem objęto wszystkie powiaty woj. opolskiego. Potrzeba realizacji projektu wynika z sytuacji demograficznej w woj. opolskim, tj. wzrostem liczby osób pow. 60 roku życia, w tym zagrożonych ubóstwem lub wykluczeniem społecznym i/lub z niepełnosprawnościami, u których występują choroby wieku podeszłego, a nie mają oni dostępu do wysokiej jakości usług zdrowotnych, poprawiających ich stan zdrowia, funkcjonalność oraz ograniczających przedwczesną umieralność. Zadania zaplanowane w projekcie mają na celu zniwelowanie powyższych barier. W projekcie przewidziano zadania: usługi telemedycyny dla 150 osób starszych (uczestników projektu) zagrożonych ubóstwem lub wykluczeniem społecznym (osób niesamodzielnych). Projekt przyczyni się do osiągnięcia celu głównego, jakim jest umożliwienie dostępu do wysokiej jakości usług zdrowotnych dla osób starszych, zagrożonych wykluczeniem społecznym, a tym samym danie im szansy na wydłużenie i poprawę jakości ich życia.
