Main objective: An increase in the availability of social services tailored to the specific needs of residents of the Municipality of Dzwola [GD] for 20 (12K and 8M); of which 2 people with disabilities) people excluded or at risk of poverty and social exclusion. (proj type. 3b-12K and 8M; 5 types of projects — 6K and 4M) [UP] through the development of environmental forms of assistance for the elderly (Senior Club in Flisy-KS) and the creation of rehabilitation equipment until 30.09.2023. GD: 20 people (12K and 8M) exclude or risk poverty and exclusion, including, but not limited to, older people, people with disabilities (including mental disorders), people in need of support in their daily functioning; included in the list of persons indicated in the Guidelines for the implementation of projects in the area of social inclusion. Results: 1. Number of social service providers supported in the programme existing after the end of Project-20 2. Number of people coming from rural areas-40 3. Number of persons with disabilities supported in programme-2 Tasks provided for in the project: 1) OUR “DRUGI DOM” — organising KS in Flisy — as part of the task, it is planned to carry out the necessary adaptation work and purchase equipment in order to guarantee the provision of social services provided for the local community. Zad.obje.employment of staff necessary.for function. FR. 2) Youth ACCESSES WITH WIEKI — realisation of cyclical stationary activities within the framework of KS — guaranteeing leisure time after the end of professional activity (e.g. cultural and educational activities); increase in activity and participation of UP in social life and physical and health activity of UP. 3) TRAVEL THAT TO LIVE!- realisation of joint outgoing activities as part of KS — the task includes the realisation of day trips for UP, the aim of which is to “detach” UP from everyday life, guaranteeing them the opportunity to take advantage of the spring break. 4) WE NEED IT RIGHT AWAY! — commissioning of equipment rental rehab., care (English) | |||

Cel główny: Wzrost dostępności usług społecznych dostosowanych do specyficznych potrzeb mieszkańców Gminy Dzwola [GD] dla 20 (12K i 8M; w tym 2 os. z niepełnosprawn.) osób wykluczonych społ. lub zagrożonych ubóstwem i wykluczeniem społ. (typ proj. 3b-12K i 8M; 5 typ proj.- 6K i 4M) [UP] poprzez rozwój środowiskowych form pomocy dla osób starszych (Klub Seniora w m. Flisy -KS) oraz utworzenie wypoż. sprzętu rehabilitacyjnego do 30.09.2023r. GD: 20 osób (12K i 8M) wyklucz.lub zagroż. ubóstwem i wyklucz.społ., w tym m.in. osoby starsze, osoby z niepełnosprawnościami (w tym z zaburzeniami psychicznymi), osoby potrzebujące wsparcia w codziennym funkcjonowaniu; mieszczące się w katalogu osób wskazanym w Wytycznych w zakresie realizacji przedsięwzięć w obszarze włączenia społ. Rezultaty: 1. Liczba wspartych w programie miejsc świadczenia usług społ. istniejących po zakończeniu proj.-20 2. Liczba osób pochodzących z obszarów wiejskich-40 3. Liczba osób z niepełnosprawnościami objętych wsparciem w programie-2 Zadania przewidziane w projekcie: 1) NASZ „DRUGI DOM”- zorganizowanie KS w m. Flisy -w ramach zadania przewidziane jest przeprowadzenie niezbędnych prac adaptacyjnych oraz zakup wyposażenia w celu zagwarantowania zaplecza do realizacji usług społecznych świadczonych dla społ. lok. Zad.obje.zatrudnie personelu niezbęd.do funkcjon. KS. 2) MŁODOŚĆ PRZYCHODZI Z WIEKIEM- realizacja cyklicznych zajęć stacjonarnych w ramach KS- gwarantujących zagospod. czasu wolnego po zakończeniu aktywności zawodowej (m.in. działalność kulturalna, edukacyjna;); zwiększenie aktywności i uczestnictwa UP w życiu społ. oraz aktywności ruchowej i zdrowotnej UP. 3) PDRÓŻOWAĆ TO ŻYĆ!- realiz. wspólnych zajęć wyjazdowych w ramach KS - zadanie obejmuje realiz. wycieczek jednodniowych dla UP, których celem jest "oderwanie" UP od życia codziennego, zagwarantowanie im możliwości skorzystania z odskoczni. 4) WSAPRCIE POTRZEBNE OD ZARAZ! - uruchomienie wypożyczalni sprzętu rehab., pielęgnacyjneg (Polish)
