Due to the growing needs in the scope of the Domestic Hospice services, an increasing number of patients with a low, fixed amount of co-financing from national sources, i.e. The National Health Fund, the Applicant assumes implementation as part of the project called the National Health Fund. “Comprehensive patient care in the framework of the activities of the Hospice in Zgierz” the following tasks: 1. Training for people around dependent people, 2. Caregiver of dependent person, 3. Physiotherapist support 4. Equipment transport service for patients, 5. Psychological and psychological support of socially excluded people. All patients who will be registered at the Home Hospice in Zgierz will receive support under the project, which will translate into a real improvement of the quality of services provided to patients of palliative and hospice care. Only a holistic approach to the patient can bring positive effects of treatment and/or pain relief. The project’s two-year duration assumes directing support for at least 200 people at risk of social exclusion and 300 people from the surroundings of dependent persons (English)

    Z uwagi na rosnące potrzeby w zakresie objęcia usługami Hospicjum Domowego coraz liczniejszej grupy pacjentów przy niskiej, stałej kwocie dofinansowania ze źródeł krajowych tj. Narodowego Funduszu Zdrowia, Wnioskodawca zakłada zrealizowanie w ramach projektu pn. :"Kompleksowa opieka pacjenta w ramach działalności Hospicjum w Zgierzu" następujących zadań: 1. Szkolenia dla osób z otoczenia osób niesamodzielnych, 2. Opiekun osoby niesamodzielnej, 3. Wsparcie fizjoterapeuty 4. Usługa transportowa sprzętu dla pacjentów, 5. Wsparcie psychologiczno-psychiatryczne osób wykluczonych społecznie. Wszyscy pacjenci, którzy zostaną zarejestrowani w Hospicjum Domowym w Zgierzu otrzymają wsparcie w ramach projektu, które przełoży się na realne podniesienie jakości świadczonych usług dla pacjentów opieki paliatywno-hospicyjnej. Wyłącznie holistyczne podejście do chorego może przynieść pozytywne efekty leczenia i/lub uśmierzania bólu. Założony dwuletni okres trwania projekt zakłada skierowanie wsparcia dla co najmniej 200 osób zagrożonych wykluczeniem społecznym oraz 300 osób z otoczenia osób niesamodzielnych (Polish)
