The main objective of the PROJECT until 31 August 2023 is an increase of 35 [20K/15M] in the number of people at risk of poverty or social exclusion in need of support in the day-to-day functioning of care services (including 25 [14K/11M] persons at home and 10 [6K/4M] specialised care services at the place of residence), residing within the meaning of the Civil Code in the voivodship of Opocztyn. The objective will be achieved by: — development of the Individual Support Path for 35 UP — support in the form of residential care services for 25 UP — support in the form of specialist care services at the place of residence for 10 UP — Meal delivery service (as a complementary activity) for 20 UP Direct result of the project will be to achieve result indicators: —Number of social service providers supported by the programme at the end of the project: 10 -Number of assisted and caring services established in the programme at the end of the project: 10, -Number of assisted and care services established in the programme after the project is completed: 5 (English) | |||||||||||||||

GŁÓWNYM CELEM PROJEKTU do 31.08.2023 r. jest wzrost o 35 [20K/15M] liczby osób zagrożonych ubóstwem lub wykluczeniem społecznym potrzebujących wsparcia w codziennym funkcjonowaniu objętych usługami opiekuńczymi (w tym 25 [14K/11M] osób usługami opiekuńczymi w miejscu zamieszkania i 10 [6K/4M] specjalistycznymi usługami opiekuńczymi w miejscu zamieszkania), zamieszkujących w rozumieniu Kodeksu cywilnego w woj.łódzkim na obszarze powiatu opoczyńskiego. Cel zostanie osiągnięty poprzez: - opracowanie Indywidualnej Ścieżki Wsparcia dla 35 UP - wsparcie w postaci usług opiekuńczych w miejscu zamieszkania dla 25 UP - wsparcie w postaci specjalistycznych usług opiekuńczych w miejscu zamieszkania dla 10 UP - usługę dowożenia posiłków (jako działanie uzupełniające) dla 20 UP Bezpośrednim rezultatem proj.będzie osiągnięcie wskaźników rezultatu: -Liczba wspartych w programie miejsc świadczenia usług społecznych istniejących po zakończeniu projektu: 10 -Liczba utworzonych w programie miejsc świadczenia usług asystenckich i opiekuńczych istniejących po zakończeniu projektu: 10, -Liczba utworzonych w programie miejsc świadczenia usług asystenckich i opiekuńczych istniejących po zakończeniu projektu: 5 (Polish)
