    Demographic change and the digitized information society present major challenges across borders. A rising number of elderly patients will want to take an active part in their health management. Well-informed people will be able to cooperate in diagnosis, therapy and recovery and meet the doctor on eye level as Empowered Patients. Health information strategies can enable them in the form of specially developed and adapted programs. Neither the presently used paper-based brochures nor online information are really suitable. New media like adaptive apps and interactive video allow for more effective and individualized information. Selfmonitoring and self-tracking apps enable patients to document and communicate their symptoms. Thus MDs can create a detailed clinical picture, and evaluate the efficiency of the therapy more easily in much more detail. Data can be collected, represented graphically, and called on effortlessly. Goals: · Expertly informed empowered patients who understand their clinical situation and know their therapy options. · Patients who can optimally prepare for interviews with doctors and actively support their own treatment including diagnosis and follow-up. · Family members who know how to support patients. · MDs able to meet well-informed patients on the same level. Measures: · Researching and interviewing doctors, patients, and family members. · Developing guidelines, educational- and self-monitoring tools. · Establishing co-creation installations, aka Living Labs, that allow for user-driven innovation. · Developing training material for medical staff to improve competences in shared therapy decision making. PROMETHEUS promotes sensible cross-border collaboration. The shared development of solutions that will be used in Demark as well as in Germany creates synergies and constitutes a win-win situation for all involved, especially in light of a quatro-helix cooperation, furthering the development of scientific excellence.
