Община Панагюрище, изпълнявайки този проект ще подобри качеството на живот и достъпа до услуги за соц. включване на хора с увреждания и на хора над 65 години, които са в невъзможност за самообслужване. Дейностите на проекта ще надградят създаденото звено към Домашен социален патронаж, финансирано по ОПРЧР, схема „Помощ в дома”, с период на изпълнение 01.01.2013-30.04.2014. В проекта е предвидено услугите да се предоставят в рамките на 18 месеца и да обхванат 75 лица с увреждания и техните семейства и лица над 65 г. с ограничения и/или невъзможност да се обслужват сами. Ще бъдат подбрани 67 лица, които ще предоставят ежедневна грижа на потребителите от целевата група в тяхната домашна среда. Предвидено е да се привлекат социален работник, медицинска сестра и психолог, които ще предоставят психологическа, социална и здравна подкрепа.Комплекса от интегрирани услуги ще се предоставя след извършването на индивидуални оценки на индивидуалните потребностите и ще се спазват хоризонталните принципи за равни възможности и недопускане на дискриминация, равенство между половете. На лицата от целевата група, които са били оценени по операция „Нови алтернативи” ще се извърши нова оценка на потребностите им, тъй като в настоящият проект услугите придобиват по своята същност интегриран и иновативен характер. Водещи принципи при изпълнението на дейностите ще е поставянето в „центъра“ нуждите и потребностите на „отделния човек” и осигуряване на интегриран достъп до здравни, социални и други услуги, според специфичните потребности на човека в неравностойно положение, предвид индивидуалната му оценка Предвидена е дейност, свързана с информираност и публичност на проекта, съобразно изискванията за прилагане на правилата за информация и комуникация 2014-2020. Ще бъдат изготвени информационни материали за разпространяване, ще се проведе 1 конференция и ще се използват и други информационни подходи като публикации в местни вестници и в сайта на Общината. The activities of the project will build on the established unit at Home Social Patronage, financed under OPHRD, scheme “Home Assistance”, with implementation period 01.01.2013-30.04.2014. The project provides for services to be provided within 18 months and to cover 75 persons with disabilities and their families and persons over 65 with restrictions and/or inability to serve themselves. 67 individuals will be selected to provide daily care to the targeted users in their home environment. It is intended to attract a social worker, nurse and psychologist who will provide psychological, social and health support. The complex of integrated services will be provided after individual assessments of individual needs and will respect the horizontal principles of equal opportunities and non-discrimination, gender equality. Targeted persons who have been assessed under Operation New Alternatives will be reassessed their needs, as the services are inherently integrated and innovative in this project. La municipalité de Panagyurishte, mettant en œuvre ce projet, améliorera la qualité de vie et l’accès aux services d’inclusion sociale des personnes handicapées et des personnes de plus de 65 ans qui sont incapables de se rendre en libre-service. Les activités du projet s’appuieront sur l’unité établie au sein du Home Social Patronage, financée dans le cadre du programme «Assistance à domicile» de l’OHRD, avec la période de mise en œuvre 01.01.2013-30.04.2014. Le projet prévoit la prestation de services dans un délai de 18 mois et couvre 75 personnes handicapées et leur famille et les personnes de plus de 65 ans avec des restrictions et/ou l’incapacité de servir elles-mêmes. 67 personnes seront sélectionnées pour fournir des soins quotidiens aux utilisateurs ciblés dans leur environnement à domicile. Il vise à attirer un travailleur social, une infirmière et un psychologue qui apporteront un soutien psychologique, social et sanitaire. Le complexe de services intégrés sera fourni après évaluation individuelle des besoins individuels et respectera les principes horizontaux d’égalité des chances et de non-discrimination, d’égalité entre les sexes. Les personnes ciblées qui ont fait l’objet d’une évaluation dans le cadre de l’opération Nouvelles solutions de rechange seront réévaluées, car les services sont intrinsèquement intégrés et novateurs dans ce projet. Les principes directeurs de la mise en œuvre des activités consisteront à placer dans le «centre» les besoins et les besoins de la «personne individuelle» et à garantir un accès intégré aux services de santé, sociaux et autres, en fonction des besoins spécifiques de la personne défavorisée, compte tenu de son évaluation individuelle, une activité liée à la sensibilisation et à la publicité du projet est prévue, conformément aux exigences relatives à l’application des règles d’information et de communication 2014-2020. Die Gemeinde Panagyurishte, die dieses Projekt durchführt, wird die Lebensqualität und den Zugang zu Dienstleistungen für die soziale Inklusion von Menschen mit Behinderungen und Menschen über 65 Jahren verbessern, die nicht in der Lage sind, sich selbst zu bedienen. Die Aktivitäten des Projekts werden auf der eingerichteten Einheit in Home Social Patronage aufbauen, die im Rahmen der OPHRD-Regelung „Heimathilfe“ mit Durchführungszeitraum 01.01.2013-30.04.2014 finanziert wird. Das Projekt sieht vor, dass innerhalb von 18 Monaten Dienstleistungen erbracht werden und 75 Menschen mit Behinderungen sowie deren Familien und Personen über 65 Personen mit Einschränkungen und/oder Unfähigkeit, sich selbst zu bedienen, abgedeckt werden. 67 Personen werden ausgewählt, um den gezielten Nutzern in ihrem häuslichen Umfeld tägliche Betreuung zu bieten. Es soll eine Sozialarbeiterin, Krankenschwester und Psychologin anlocken, die psychologische, soziale und gesundheitliche Unterstützung leisten wird. Der Komplex integrierter Dienstleistungen wird nach individueller Einschätzung der individuellen Bedürfnisse erbracht und wird die horizontalen Grundsätze der Chancengleichheit und Nichtdiskriminierung, der Gleichstellung der Geschlechter beachten. Zielpersonen, die im Rahmen der Operation „Neue Alternativen“ bewertet wurden, werden ihre Bedürfnisse neu bewertet, da die Dienstleistungen in diesem Projekt von Natur aus integriert und innovativ sind. Leitprinzipien bei der Durchführung der Maßnahmen werden in den Mittelpunkt der Bedürfnisse und Bedürfnisse der „individuellen Person“ und der Gewährleistung eines integrierten Zugangs zu Gesundheits-, Sozial- und sonstigen Dienstleistungen entsprechend den besonderen Bedürfnissen der benachteiligten Person stehen, da ihre individuelle Bewertung eine Tätigkeit im Zusammenhang mit dem Bewusstsein und der Öffentlichkeit des Projekts vorsieht, die den Anforderungen für die Anwendung der Vorschriften für die Information und Kommunikation 2014-2020 entspricht. De gemeente Panagyurishte, die dit project uitvoert, zal de kwaliteit van leven en de toegang tot diensten voor sociale inclusie verbeteren voor mensen met een handicap en personen ouder dan 65 jaar die niet in staat zijn zichzelf te bedienen. De activiteiten van het project zullen voortbouwen op de opgerichte eenheid van Home Social Patronage, gefinancierd in het kader van OPHRD, regeling „Home Assistance”, met uitvoeringsperiode 01.01.2013-30.4.2014. Het project voorziet in diensten die binnen 18 maanden worden verleend en betrekking hebben op 75 personen met een handicap en hun gezinnen en personen ouder dan 65 jaar met beperkingen en/of onvermogen om zichzelf te dienen. 67 personen worden geselecteerd om dagelijkse zorg te bieden aan de beoogde gebruikers in hun thuisomgeving. Het is bedoeld om een sociaal werker, verpleegkundige en psycholoog aan te trekken die psychologische, sociale en gezondheidsondersteuning zal bieden. Het complex van geïntegreerde diensten zal worden verstrekt na individuele beoordelingen van individuele behoeften en zal de horizontale beginselen van gelijke kansen en non-discriminatie, gendergelijkheid eerbiedigen. De beoogde personen die in het kader van operatie New Alternatives zijn beoordeeld, zullen hun behoeften opnieuw beoordelen, aangezien de diensten inherent geïntegreerd en innovatief zijn in dit project. Il comune di Panagyurishte, attuando questo progetto, migliorerà la qualità della vita e l'accesso ai servizi per l'inclusione sociale delle persone con disabilità e delle persone di età superiore a 65 anni che non sono in grado di self-service. Le attività del progetto si baseranno sull'unità istituita presso l'Home Social Patronage, finanziata nell'ambito dell'OPHRD, regime "Assistenza domestica", con il periodo di attuazione 1º.1.2013-30.4.2014. Il progetto prevede servizi da fornire entro 18 mesi e copre 75 persone con disabilità e le loro famiglie e le persone di età superiore a 65 anni con restrizioni e/o incapacità di servirsi. 67 persone saranno selezionate per fornire assistenza quotidiana agli utenti interessati nel loro ambiente domestico. L'obiettivo è quello di attrarre un assistente sociale, infermiere e psicologo che fornirà sostegno psicologico, sociale e sanitario. Il complesso di servizi integrati sarà fornito dopo valutazioni individuali delle esigenze individuali e rispetterà i principi orizzontali di pari opportunità e non discriminazione, parità di genere. Le persone destinatarie che sono state valutate nell'ambito dell'operazione "Nuove alternative" saranno rivalutate le loro esigenze, in quanto i servizi sono intrinsecamente integrati e innovativi in questo progetto. I principi guida nell'attuazione delle attività consisteranno nel porre al "centro" le esigenze e le esigenze della "persona individuale" e nel garantire un accesso integrato ai servizi sanitari, sociali e di altro tipo, in funzione delle esigenze specifiche della persona svantaggiata, tenuto conto della sua valutazione individuale, è prevista un'attività di sensibilizzazione e pubblicità del progetto, conformemente ai requisiti per l'applicazione delle norme in materia di informazione e comunicazione 2014-2020. Saranno prodotti materiali informativi per la diffusione, si terrà una conferenza e si utilizzeranno altri approcci informativi, come le pubblicazioni sui giornali locali e sul sito web comunale. El municipio de Panagyurishte, ejecutando este proyecto, mejorará la calidad de vida y el acceso a los servicios para la inclusión social de las personas con discapacidad y las personas mayores de 65 años que no pueden autoservicio. Las actividades del proyecto se basarán en la unidad establecida en el Patronato Social del Hogar, financiada en el marco de la OPHRD, esquema «Asistencia al hogar», con el período de ejecución 01.01.2013-30.04.2014. El proyecto prevé la prestación de servicios en un plazo de 18 meses y la cobertura de 75 personas con discapacidad y sus familias y mayores de 65 años con restricciones o incapacidad para servirse. Se seleccionarán 67 personas para prestar atención diaria a los usuarios destinatarios en su entorno familiar. Su objetivo es atraer a un trabajador social, enfermero y psicólogo que prestará apoyo psicológico, social y sanitario. El complejo de servicios integrados se prestará después de evaluaciones individuales de las necesidades individuales y respetará los principios horizontales de igualdad de oportunidades y no discriminación, igualdad de género. Las personas seleccionadas que hayan sido evaluadas en el marco de la Operación Nuevas Alternativas serán reevaluadas sus necesidades, ya que los servicios son inherentemente integrados e innovadores en este proyecto. Los principios rectores en la ejecución de las actividades serán situar en el «centro» las necesidades y necesidades de la «persona individual» y garantizar el acceso integrado a los servicios sanitarios, sociales y de otro tipo, en función de las necesidades específicas de la persona desfavorecidas, habida cuenta de su evaluación individual, se prevé una actividad relacionada con la sensibilización y la publicidad del proyecto, de conformidad con los requisitos de aplicación de las normas de información y comunicación 2014-2020.
