The aim of the project is to create and develop a model of integral assistance (social care and care services) at home in Širvintos district in order to provide comprehensive assistance to disabled and elderly people and help family members caring for their relatives to reconcile family and work responsibilities. Integrated assistance is planned for 35 elderly people and persons with disabilities. During the implementation of the project, 2 mobile teams will employ nursing specialists — 1 nurse, 6 nurse assistants and 1 masseur, social services specialists — 1 social worker, 5 social worker assistants. Individuals caring for relatives at home will be provided with individual counselling to provide them with the necessary knowledge, skills in care and care for the elderly and disabled people. Individual consultation is foreseen for 20 family members. Training for all team members — 14 people — will ensure the quality of the integrated service and consultation. Projekto tikslas - sukurti ir plėtoti integralios pagalbos (socialinės globos ir slaugos paslaugų) namuose modelį Širvintų rajone, siekiant teikti kompleksinę pagalbą neįgaliems ir senyvo amžiaus asmenims bei padėti šeimos nariams, prižiūrintiems savo artimuosius, derinti šeimos ir darbo įsipareigojimus. Integralią pagalbą planuojama teikti 35 senyvo amžiaus asmenims ir asmenims su negalia. Projekto įgyvendinimo metu 2 mobiliose komandose bus įdarbinti slaugos specialistai - 1 slaugytojas, 6 slaugytojo padėjėjai ir 1 masažuotojas, socialinių paslaugų specialistai - 1 socialinis darbuotojas, 5 socialinio darbuotojo padėjėjai. Asmenims, prižiūrintiems artimuosius namuose, bus teikiamos individualios konsultacijos, siekiant suteikti jiems reikalingų žinių, įgūdžių senyvo amžiaus ir neįgalių asmenų priežiūros bei slaugos srityse. Individualiai konsultuoti numatoma 20 šeimos narių. Kokybišką integralios paslaugos teikimą ir konsultavimą užtikrins organizuoti mokymai visiems komandos nariams - 14 asmenų. Projekto veiklomis taip pat bus skatinama neformali pagalba, įtraukiant į integralios pagalbos teikimo procesą savanorius ir bendruomenės narius - 10 asmenų. (Lithuanian)
