By introducing a family life plan at Hanzeheerd, it is much better to respond to the individual wishes and needs of the individual client. Informal carers, clients and employees see working with a family life plan as an added value in relation to personal care. In order to optimise the choice of clients, Hanzeheerd has also started with the introduction of the Intramurale Personal Subsequence (IPVB). Residents can create their own package of care and well-being based on their care-gravity package (ZZP). This process does not look at the process of introducing the family life plan and the IPVB, but focuses on the different organisation of work, both individually and in a team. Door het invoeren van een familieleefplan bij Hanzeheerd kan veel beter worden ingespeeld op de individuele wensen en behoeften van de individuele cliënt. Mantelzorgers, cliënten en medewerkers zien het werken met een familieleefplan als een meerwaarde in relatie tot persoonsgerichte zorg. Om de keuzevrijheid van cliënten te optimaliseren is Hanzeheerd eveneens gestart met de invoering van de Intramurale Persoonsvolgende bekostiging (IPVB). Bewoners kunnen op basis van hun zorgzwaartepakket (ZZP) een eigen arrangement aan zorg en welzijn samenstellen. Met dit traject wordt niet gekeken naar het proces van invoeren van het familieleefplan en de IPVB, maar ligt de nadruk op het anders organiseren van werk, zowel individueel als in teamverband.
