Проекта предвижда създаване на Център за подкрепа на лица с увреждания и техните семейства, вкл. с тежки множествени увреждания на територията на община Кюстендил. В центъра ще се създадат условия и предпоставки за инвестиране в комплексни интегрирани мерки, насочени към подобряването на достъпа до социални и здравни услуги на лицата с увреждания, вкл. с тежки множествени увреждания, както и ще се осигури подкрепа за техните семейства, които полагат грижите за тези лица.В Центъра ще се предоставят комплексни услуги и качествена грижа за лицата с увреждания, вкл. с тежки увреждания, чрез подкрепата за развитие и комплексно обслужване на тези лица и техните семейства. Успоредно с това, лицата, полагащи грижи за зависими членове на семейството ще получат подкрепа чрез мерки, свързани с улесняване достъпа им до пазара на труда и връщането им на реалния пазар на труда, както и психологическа подкрепа. Центъра за подкрепа на лица с увреждания и техните семейства, вкл. с тежки множествени увреждания ще осигури заместваща грижа, дневни, полудневни и почасови услуги като грижа, рехабилитация, терапия, консултиране, информиране и подкрепа, включително и на лицата, полагащи грижи за тях в домашна среда, мобилни услуги и други подкрепящи дейности, съгласно спецификата и потребностите на целевата група. The project provides for the establishment of a Centre for Support of Persons with Disabilities and their Families, including with severe multiple disabilities on the territory of Kyustendil Municipality. The center will create conditions and conditions for investment in complex integrated measures aimed at improving access to social and health services for persons with disabilities, including with severe multiple disabilities, as well as providing support for their families who care for these persons. The Center will provide comprehensive services and quality care for persons with disabilities, including severe disabilities, through support for development and comprehensive service to these persons and their families. In parallel, carers of dependent family members will receive support through measures related to facilitating their access to the labour market and their return to the real labour market, as well as psychological support. The Support Centre for persons with disabilities and their families, including with severe multiple disabilities, will provide replacement care, day care, half-day and hourly services such as care, rehabilitation, therapy, counselling, information and support, including their home caregivers, mobile services and other supporting activities, according to the specifics and needs of the target group. Le projet prévoit la création d’un centre de soutien aux personnes handicapées et à leur famille, y compris les personnes gravement handicapées multiples sur le territoire de la municipalité de Kyustendil. Le centre créera les conditions et les conditions d’investissement dans des mesures intégrées complexes visant à améliorer l’accès aux services sociaux et de santé pour les personnes handicapées, y compris les personnes souffrant de handicaps multiples graves, ainsi qu’à fournir un soutien à leurs familles qui s’occupent de ces personnes. Das Projekt sieht die Einrichtung eines Zentrums zur Unterstützung von Menschen mit Behinderungen und ihrer Familien vor, unter anderem mit schweren Mehrfachbehinderungen auf dem Gebiet der Gemeinde Kyustendil. Das Zentrum wird Bedingungen und Bedingungen für Investitionen in komplexe integrierte Maßnahmen schaffen, die darauf abzielen, den Zugang zu Sozial- und Gesundheitsdiensten für Menschen mit Behinderungen, auch mit schweren Mehrfachbehinderungen, zu verbessern und ihre Familien zu unterstützen, die sich um diese Personen kümmern. Parallel dazu erhalten Betreuer von unterhaltsberechtigten Familienangehörigen Unterstützung durch Maßnahmen, die darauf abzielen, ihren Zugang zum Arbeitsmarkt und ihre Rückkehr zum Arbeitsmarkt sowie psychologische Unterstützung zu erleichtern. Das Unterstützungszentrum für Menschen mit Behinderungen und deren Familien, auch mit schweren Mehrfachbehinderungen, bietet Ersatzversorgung, Tagesbetreuung, Halbtages- und Stundendienste wie Pflege, Rehabilitation, Therapie, Beratung, Information und Unterstützung, einschließlich ihrer Hauspfleger, mobile Dienste und andere unterstützende Tätigkeiten, je nach den Besonderheiten und Bedürfnissen der Zielgruppe.
