Celem głównym proj jest aktywizacja społeczno - zawodowa 30 osób niepełnosprawnych (18K, 12 M) zagrożonych ubóstwem lub wykluczeniem społecznym, zamieszkujących w myśl KC woj. Warmińsko - mazurskie w gminach, w których procent osób korzystających ze świadczeń pomocy społecznej jest wyższy niż średni procent osób korzystających ze świadczeń pomocy społecznej na terenie województwa, poprzez udział w kompleksowym wsparciu do 01.05.2019. W ramach projektu założono osiągnięcie następujących wskaźników: • Liczba osób zagrożonych ubóstwem lub wykluczeniem społecznym objętych wsparciem w programie – 30 • Liczba osób z niepełnosprawnościami objętych wsparciem w programie - 30 • Liczba osób zagrożonych ubóstwem lub wykluczeniem społecznym, które uzyskały kwalifikacje po opuszczeniu programu – 10 • Liczba osób zagrożonych ubóstwem lub wykluczeniem społecznym pracujących po opuszczeniu programu (łącznie z pracującymi na własny rachunek) – 8 (min. 25%). Grupę docelową stanowi 30 osób niepełnosprawnych (18K, 12M) zagrożonych ubóstwem lub wykluczeniem społ. Będą to osoby korzystające ze świadczeń z pomocy społ. zg. z ustawą z 12.03.2004 r. o pomocy społ. lub kwalifikujące się do objęcia wsparciem pomocy społ., zamieszkałe w myśl KC następujące gminy: • Powiat Bartoszycki: Bisztynek, Górowo Iławeckie (miasto), Górowo Ił.. (gmina wiejska), Sępopol, Bartoszyce (gmina wiejska). • Powiat Braniewski: Frombork, Pieniężno, Braniewo (gmina wiejska), Lelkowo, Płoskinia, Wilczęta. • Powiat Kętrzyński: Korsze, Reszel, Barciany, Kętrzyn (gmina wiejska), Srokowo. W ramach projektu zostaną zrealizowane takie zadania, jak: 1. Aktywizacja społeczna 2. Aktywizacja zdrowotna 3. Aktywizacja edukacyjna 4. Aktywizacja zawodowa. The main objective of the project is to activate the socio-occupational activity of 30 people with disabilities (18K, 12 M) at risk of poverty or social exclusion, living in accordance with the KC voivodships. Warmińsko-Mazurskie in communes where the percentage of people benefiting from social assistance benefits is higher than the average percentage of people benefiting from social assistance in the voivodship, through participation in comprehensive support until 01.05.2019. The project aims to achieve the following indicators: • Number of people at risk of poverty or social exclusion supported in the programme – 30 • Number of people with disabilities supported in the programme – 30 • Number of people at risk of poverty or social exclusion who have obtained qualifications after leaving the programme – 10 • Number of people at risk of poverty or social exclusion after leaving the programme (including self-employed) – 8 (min. 25 %). The target group consists of 30 people with disabilities (18K, 12M) at risk of poverty or social exclusion. These will be persons benefiting from the social assistance provided in accordance with the Act of 12.03.2004 on social aid or eligible for community aid, residing in accordance with the CCC of the following municipalities: • Bartoszyce district: Bisztynek, Górowo Iławeckie (city), Górowo Ił. (rural commune), Sępopol, Bartoszyce (rural commune). • Braniewski County: Frombork, Money, Braniewo (rural commune), Lelkowo, Płoskinia, Wolf. • Kętrzyn County: Korsze, Reszel, Barciany, Ketrzyn (rural commune), Srokowo. L’objectif principal du projet est l’activation socioprofessionnelle de 30 personnes handicapées (18K, 12 M) menacées de pauvreté ou d’exclusion sociale, vivant conformément au CC. Warmińsko-Mazurskie dans les municipalités, où le pourcentage de personnes bénéficiant de prestations d’assistance sociale est plus élevé que le pourcentage moyen de personnes bénéficiant de prestations d’assistance sociale dans la voïvodie, en participant à un soutien global jusqu’au 1er mai 2019. Le projet vise à atteindre les indicateurs suivants: • Nombre de personnes exposées au risque de pauvreté ou d’exclusion sociale soutenues dans le programme — 30 • Nombre de personnes handicapées soutenues dans le programme — 30 • Nombre de personnes exposées au risque de pauvreté ou d’exclusion sociale ayant obtenu une qualification après leur départ — 10 • Nombre de personnes exposées au risque de pauvreté ou d’exclusion sociale au travail après avoir quitté le programme (y compris les travailleurs indépendants) — 8 (min. 25 %). Le groupe cible est de 30 personnes handicapées (18K, 12M) menacées de pauvreté ou d’exclusion sociale. Il s’agit des personnes bénéficiant de l’aide sociale en vertu de la loi du 12 mars 2004 sur l’assistance sociale ou ayant droit à l’aide sociale, résidant conformément au code des douanes des municipalités suivantes: • Quartier de Bartoszycki: Bisztynek, Górowo Iławeckie (ville), Górowo Ił.. (municipalité de village), Sępopol, Bartoszyce (municipalité de village). • District de Braniewski: Frombork, Money, Braniewo (commune village), Lelkowo, Płoskinia, Wilczęta. • District de Kętrzyński: Korsha, Reszel, Barciany, Kętrzyn (municipalité de village), Srokowo.
