Przedmiotem projektu jest podjęcie działań skutkujących poprawą stanu zdrowia osób zagrożonych ubóstwem i wykluczeniem społecznym z gminy Korfantów i okolic: osób starszych i niepełnosprawnych. W ramach projektu będą świadczone usługi zdrowotne w środowisku domowym oraz w Opolskim Centrum Rehabilitacji w dziennej formie. Zostanie zorganizowana wypożyczalnia sprzętu rehabilitacyjnego, a także będą organizowane działanie edukacyjne: szkolenia dla opiekunów faktycznych w zakresie opieki medycznej oraz dla kadry medycznej – w celu podniesienia kompetencji i poprawy jakości świadczonych usług. W projekcie preferowane będą osoby ze wskazaniem lekarskim bezpośrednio lub do pół roku po odbytej hospitalizacji, spełniające warunki rekrutacji. W dziennej formie pobytu zostanie utworzone 10 miejsc opieki, a do 6 pacjentów dziennie pracownicy OCR będą dojeżdżać (zostanie uruchomiona grupa wyjazdowa). Projekt stanowi odpowiedź na dostrzeżone problemy społeczne: -przyspieszony proces starzenia się ludności (wzrost liczby jednoosobowych gospodarstw domowych starszych osób samotnie mieszkających o utrudnionym dostępie do usług zdrowotnych) i problem związany ze wzrostem obciążeń ekonomicznych rodzin, -trudność lub niemożność samodzielnego wykonywania podstawowych czynności życiowych (wg wskazań skali Barthel) i konieczność podjęcia działań usprawniających u osób niepełnosprawnych, niesamodzielnych, szczególnie w wieku 65+, -niewielki zakres zabezpieczenia potrzeb starszych mieszkańców w zakresie miejsc pobytu w dziennych domach pomocy przy systematycznym wzroście ich liczby. W efekcie projektu zostanie zintensyfikowana pomoc pielęgniarsko-medyczna, w tym rehabilitacja i fizjoterapia, terapia zajęciowa, będą prowadzone działania, prowadzące do usprawnienia i zwiększenia komfortu życia tych osób. Pacjenci będą mieli mieli zapewnione bezpłatne, kompleksowe usługi zdrowotne, transport, wyżywienie. (Polish)

The subject of the project is to take measures to improve the health of people at risk of poverty and social exclusion from the Korfantów commune and the surrounding areas: the elderly and the disabled. The project will provide health services in the home environment and in the Opolskie Centre for Rehabilitation in a daily form. A rental of rehabilitation equipment will be organised and an educational activity will be organised: training for carers in the field of medical care and medical staff – in order to improve competences and improve the quality of services provided. The project will prefer people with medical indications directly or up to six months after hospitalisation, fulfilling the conditions for recruitment. 10 places of care will be created in the daily form of stay, and up to 6 patients per day will be commuted by OCR staff (outbound group will be activated). The project responds to the perceived social problems: —accelerated process of ageing population (increased number of single-person households living alone with difficult access to health services) and the problem of increasing the economic burden of families, -difficulty or inability to perform basic life activities independently (according to the Barthel scale) and the need to take measures to improve disabled persons, especially at 65+, -small coverage of the needs of older residents in terms of living places in daily homes. As a result of the project, nursing and medical assistance will be intensified, including rehabilitation and physiotherapy, occupational therapy, activities will be carried out, leading to improvement and increased comfort of life of these people. Patients will be provided free of charge, comprehensive health services, transport and food. (English)

L’objectif du projet est de prendre des mesures pour améliorer l’état de santé des personnes exposées au risque de pauvreté et d’exclusion sociale de la municipalité de Korfant et des environs: personnes âgées et handicapées. Le projet fournira quotidiennement des services de santé à domicile et au centre de réadaptation d’Opole. Il y aura une location de matériel de réhabilitation, ainsi qu’une activité éducative: formation des soignants effectifs dans le domaine des soins médicaux et du personnel médical — afin d’améliorer les compétences et la qualité des services fournis. Dans le projet, les personnes ayant une indication médicale seront préférées soit directement, soit jusqu’à un demi-an après l’hospitalisation, répondant aux conditions de recrutement. 10 lieux de soins seront créés dans la forme quotidienne de séjour et jusqu’à 6 patients par jour seront transportés par le personnel de l’OCR (un groupe de départ sera lancé). Le projet répond aux problèmes sociaux perçus: — le processus accéléré du vieillissement (augmentation du nombre de ménages monoparentaux de personnes âgées vivant seules ayant un accès difficile aux services de santé) et le problème de l’augmentation de la charge économique pour les familles, — la difficulté ou l’incapacité de mener de manière autonome des activités de base (comme l’indique l’échelle Barthel) et la nécessité de mesures d’amélioration pour les personnes handicapées et les personnes dépendantes, en particulier à l’âge de 65 ans et plus, — la faible proportion des besoins des personnes âgées en termes de lieux de résidence dans des foyers quotidiens, avec une augmentation systématique de leur nombre. Grâce au projet, les soins infirmiers et l’assistance médicale, y compris la réadaptation et la physiothérapie, l’ergothérapie, seront renforcés, ce qui permettra d’améliorer et d’améliorer le bien-être de ces personnes. Les patients recevront gratuitement des services de santé complets, des transports, des repas. (French)
