Подкрепата ще бъде насочена към предоставяне на интегрирани здравно- социални услуги в домашна среда. Предоставянето на почасови интегрирани здравно- социални услуги ще се осъществява съобразно разработената от Министерство на здравеопазването Методика за предоставяне на патронажни грижи по домовете за възрастни хора и хора с увреждания , която е създадена по проект BG05M9OP001-2.028-01 "Патронажна грижа за възрастни хора и лица с увреждания- Компонент 1" и на територията на община Годеч, включваща гр. Годеч и всичките 19 села. Всички медицински специалисти и специалисти в областта на социалните услуги, които ще са ангажирани с предоставянето на патронажни грижи, следва да преминат специализирано обучение по програмата, разработена от Министерство на здравеопазването. Всяко лице от целевата група може да получава здравно- социални услуги до 2 часа на ден. В резултат на изпълнение на дейностите ще се подобри качеството на живот и възможностите за социално включване на хората с увреждания и възрастните хора, чрез осигуряване на мрежа от услуги в домашна среда и изграждане на кадрови капацитет за предоставянето им. Специфичната цел на операцията е да се изгради модел за патронажни грижи за възрастните хора и лица с увреждания, включително с хронични заболявания и трайни увреждания с цел осигуряване на почасови здравно- социални услуги в техните домове. Внедряването на този модел е в съответствие с Плана за действие за периода 2018- 2021 г. за изпълнение на Националната стратегия за дългосрочна грижа, одобрен с РМС № 28/ 19.01.2018 г. Операцията ще осигури надграждане на създадения модел за предоставяне на почасови здравно- социални услуги, които ще надградят резултатите по проект "Намаляване на бедността и насърчаване на социалното включване". Support will focus on the provision of integrated health-social services in the home environment. The provision of hourly integrated health-social services will be carried out in accordance with the Methodology for the provision of patronage care in the homes for elderly and disabled people developed by the Ministry of Health, which was established under project BG05M9OP001-2.028-01 “Patronage care for elderly and disabled persons — Component 1” and on the territory of the municipality of Godech, including the town of Godech and all 19 villages. All medical and social services professionals who will be involved in the provision of patronage care should receive specialised training under the programme developed by the Ministry of Health. Any person in the target group can receive health and social services up to 2 hours a day. As a result of the implementation of the activities, the quality of life and opportunities for social inclusion of people with disabilities and the elderly will be improved by providing a network of services in the home environment and building staff capacity to provide them. The specific objective of the operation is to build a model of patronage care for the elderly and persons with disabilities, including chronic diseases and permanent disabilities, in order to provide hourly health-social services in their homes. The implementation of this model is in line with the Action Plan for the period 2018-2021 for the implementation of the National Strategy for Long-term Care, approved by Council of Ministers Decree No 28/19.01.2018. The operation will provide an upgrade of the established model for the provision of hourly health and social services, which will build on the results of the project “Reducing poverty and promoting social inclusion”.
