В рамките на проект "Патронажна грижа в община Казанлък" ще се предоставят почасови мобилни интегрирани здравно-социални услуги на нуждаещите се лица с увреждания и възрастните хора, живеещи на територията на община Казанлък. Целта на проекта е да се подобри качеството на живот и възможностите за социално включване на лицата от целевата група по проекта, осигурявайки пакет от дейности/услуги по домовете им от квалифицирани специалисти. По този начин ще се подкрепят хора с увреждания и възрастни хора, които имат разнообразни и динамични потребности в ежедневието си. В рамките на настоящия проект, мобилните интегрирани здравно-социални услуги ще се предоставят на 142 потребителя (лица с увреждания и възрастни хора, вкл. с хронични заболявания и трайни увреждания). На всеки един от потребителите услугата ще е насочена към индивидуалните им водещи потребности в дейности, в които те срещат затруднение и се нуждаят от подкрепа.За реализация на проекта ще бъде назначен персонал за организиране и управление на услугата, приложни специалисти за предоставяне на патронажната грижа, помощен персонал, консултанти и обучители. Ще бъде закупено МПС, материали и консумативи и наето оборудване за обезпечаване на самата интегрирана услуга.Интегриганата здравно-социална услуга ще се предоставя съгласно Методика, разработена от Министерство на здравеопазването. The project “Patronage Care in Kazanlak Municipality” will provide hourly mobile integrated health and social services to people in need with disabilities and elderly people living in the municipality of Kazanlak. The aim of the project is to improve the quality of life and the opportunities for social inclusion of the target group of the project by providing a package of activities/services at home by qualified specialists. This will support people with disabilities and older people who have diverse and dynamic needs in their daily lives. Within the framework of this project, mobile integrated health-social services will be provided to 142 users (persons with disabilities and elderly people, including chronic diseases and permanent disabilities). For each of the users, the service will address their individual leading needs in activities where they encounter difficulties and need support. For the realisation of the project will be assigned staff for organising and managing the service, applied specialists for providing patronage care, support staff, consultants and trainers. Motor vehicles, materials and consumables and hired equipment for the provision of the integrated service will be purchased.The integrated health-social service will be provided in accordance with the Methodology developed by the Ministry of Health.
