The proposed project aims at improving the quality of life and access to social inclusion services, including health services, in response to the complex needs of persons with disabilities, people over 65 years old, unable to self-service and elderly people at risk from the municipality of Sredets, in order to overcome social exclusion and poverty. The provision of quality part-time integrated services in the community and in the home environment is a mechanism for preventing the risk of falling into an institution. At the same time, it creates conditions for the acquisition of skills for independent living, tailored to the physical and social status of consumers. The project will expand the activities of the Center for Social and Health Services — CSA, established under the Independent Life Scheme of the HRD OP. The project fully overlaps the objectives of the scheme “LAG Sredets” — FOR BEST LIFE OF THE TERRITORY and will respond to the need to provide assistance to the target group to organise their own lifestyles and daily activities, as well as the lack of social contacts. For the provision of the hourly services within the CSE, persons will be employed to serve those in need on the territory concerned. The team of the CSE will include 16 persons. The provision of services will be based on the identified needs and problems and the individual social assessment of the target group representatives in need of constant care and inability to self-service. We plan the project to serve 50 people. The personal assistant service will be provided in the homes of consumers. For a better quality provision of services, we foresee that the complex of social, health, psychological and rehabilitation services will be provided on an hourly basis, both in the CSE and at the homes of consumers. Sredets Municipality has a specialised car with a ramp for people with disabilities, purchased with funds provided by the Independent Life Scheme of the HRD OP, which will transport consumers from their homes to the CSU and vice versa. Предложеният проект е насочен към подобряване качеството на живот и достъпа до услуги за социално включване, включително здравни, в отговор на комплексните потребности на лица с увреждания, хора над 65 г. в невъзможност за самообслужване и възрастни хора в риск от община Средец, с цел преодоляване на социалното изключване и бедността. Предоставянето на качествени почасови интегрирани услуги в общността и в домашна среда е механизъм за превенция на риска от попадане в институция. Едновременно с това се създават условия за усвояване на умения за независим живот, съобразени с физическия и социален статус на потребителите. Проектът ще разшири дейността на Център за социални и здравни услуги - ЦСЗУ, създаден по схема "Независим живот" на ОП "РЧР". Проектът напълно припокрива целите на схемата „МИГ СРЕДЕЦ“ – ЗА ПО – ДОБЪР ЖИВОТ НА ТЕРИТОРИЯТА“ и ще отговори на необходимостта от предоставяне на помощ на лицата от целевата група да организират самостоятелно бита си и ежедневните дейности, както и липсата на социални контакти. За предоставяне на почасовите услуги в рамките на ЦСЗУ ще бъдат наети лица, които да обслужват нуждаещите се на съответната територия. Екипът на ЦСЗУ ще включва 16 лица. Предоставянето на услугите ще се основава на идентифицираните нужди и проблеми и индивидуалната социална оценка на представителите на целевата група, нуждаещи се от постоянна грижа и в невъзможност за самообслужване. Предвиждаме проекта да обслужва 50 лица. Услугата - личен асистент ще бъде предоставяна в домовете на потребителите. За по-качествено предоставяне на услугите предвиждаме комплексът от социални, здравни, психологически и рехабилитационни услуги да се предоставят почасово, както в ЦСЗУ, така и по домовете на потребителите. Община Средец разполага със специализиран автомобил с рампа за хора с увреждания, закупен със средства, осигурени от схема Независим живот на ОП "РЧР", с който ще бъдат извозвани потребителите от домовете им до ЦСЗУ и обратно.
